Incentive pay 1.5 times hourly wage over 8 hours, plus $hourly pick up avaialble.

JOB SUMMARY (Ask about Sign-On Bonus and PM/NOC shift differentials + Pick Up Pay):


An individual trained to perform personal cares for the resident under the supervision of a licensed nurse.

Directly responsible to the RN/LPN team leader and nursing supervisory staff.


ESSENTIAL JOB FUNCTIONS:

1. Maintain and actively promote effective communication with residents and

family members / personal representatives.

2. Responsible for maintaining a positive image of LHHS in the community keeping in alignment with our mission, vision, & values.

3. Perform care as directed by the RN care coordinator.

4. Promptly notify the RN care coordinator of any change in resident status.

5. Provide services consistent with the resident service plan.

6. Participate in required initial and ongoing training programs and complete 15 hours of in-service training per year.

7. Use proper body mechanics in the performance of all tasks.

8. Only perform tasks in which training has been provided.

9. Always practice standard precautions.

10. Document services in an accurate and timely manner.

11. Cooperate with scheduling to accomplish adequate coverage for resident services.

12. Maintain a calm, composed attitude with compassion and empathy, while maintaining appropriate boundaries.

13. Work cooperatively with all disciplines and treat residents, families and other staff with dignity and respect.

14. Performs other similar duties, as needed.

Requirements:

QUALIFICATIONS

1. Commitment to quality programs and services for all residents.

2. Excellent communication and human relation skills.

3. Ability to relate well to the residents, families, staff, churches and the community in general.

4. Ability to maintain and protect the confidentiality of information.

5. Ability to exercise independent judgment, make sound decisions and exhibit flexibility when needed.

6. Clear background check.

7. Physically able to care for residents’ needs.

EXPERIENCE AND TRAINING:

1. CNA registry certification in good standing or ability to obtain in the State of Wisconsin.

2. Has not had a lapse of 24 consecutive months where the aide had not provided services for compensation.

3. CBRF Training as outlined in HFS-83
