JHS, Inc. operates a 55 bed nursing home, 12 bed hospital and provider clinic. We are centrally located between Atlanta and Macon, in Monticello, GA. $5,000 sign on bonus offered for full-time employment. We are a small facility that is team oriented and values their employees.

The Licensed Practical Nurse (LPN) is responsible for age appropriate assessment, planning, implementation, and evaluation of residents. Responsible to assure the planned care is delivered in a safe, economical and efficient manner, which follows facility’s policies and procedures. Full-time hires are eligible for a full line of insurance products, retirement, paid time off, etc.

GENERAL PURPOSE AND ESSENTIAL FUNCTIONS:

Responsibilities and Duties:

  1. Receive report and count with off-going nurse. Be prepared at the end of the shift to give report to the on-coming nurse unless you are in an emergency.
  2. Comply with State and Federal Rules and Regulations for long term care facilities; as well as follow facility policies and procedures.
  3. Supervise, teach, and assist C.N.A.s as necessary. Note that all residents are being turned and positioned, cleaned and dried, etc.
  4. Acquire general knowledge of pertinent facility and nursing policies and other department’s functions and interactions with nursing service.
  5. Function as an important member of the nursing team, receiving assignment from and reporting to supervisor(s) as indicated.
  6. Does work accurately, neatly, and completely.
  7. Maintains organization at the nurse’s station.
  8. Communicates resident concerns, problems, and / or changes in condition to the physician.
  9. Provides privacy for all residents.
  10. Complete admissions, discharges, and transfers of resident forms as needed.
  11. Answers resident’s call light and seeks assistance as necessary.
  12. Observes and reports patient’s general condition and behavior, both physical and mental.
  13. Administer medications as ordered with each medication pass and document in the EHR.
  14. If indicated, complete treatments as ordered and document in the EHR.
  15. Checks to assure the C.N.A.s have completed all duties and POC by the end of the shift.
  16. Document all pertinent information in the EHR.
  17. Complete incident reports, event fall forms, and observation forms in the EHR each shift and report to DON/ADON as necessary.
  18. Knows when and how to administer CPR and does so.
  19. Complete Report Sheet and Charge Nurse Checklist each shift.
  20. Update the schedule and daily assignment as needed.
  21. Assist physicians when at facility and via phone.
  22. Ensure 2 step PPD is done on admission and results documented on the EHR.
  23. Orders OTC medications as needed and keeps med room organized.
  24. Assist the DON, ADON, and Administrator with duties and projects as assigned and maintain confidentiality of all records and issues in the facility.
  25. Receive visitors, answer telephone, and answer call lights in a courteous manner.
  26. Fax pharmacy reorder sheets as needed.
  27. Assure that all residents receive meals and are assisted with their meals as needed.
  28. Receive, verify, and put away all medications delivered by pharmacies.
  29. Responsible for communicating with the doctors and family members regarding changes in residents.
  30. Responsible for recording I & O’s in the EHR as ordered.
  31. Responsible for carrying out all physician orders in a timely manner and notifying physician if there is a reason the order cannot be carried out.
  32. Responsible for documenting glucose levels in the EHR and in the accu check book and notifying physicians as ordered.
  33. Stock medication carts at the end of your shift.

Related Functions:

  1. Arrives for the scheduled shift no earlier than fifteen (15) minutes before the shift begins and clocks out no later than fifteen (15) minutes after the scheduled shift ends.
  2. Required to work weekends on a rotating basis.
  3. Responsible for one overtime shift a month if needed.
  4. Demonstrates loyalty to the organization, honesty, and integrity at all times in the care and use of patient / resident and facility property.
  5. Demonstrates positive support of any and all programs / projects implemented by the DON or Administration.
  6. Maintains confidentiality of patient / resident information.
  7. Displays a neat, clean, and professional appearance at all times.
  8. Cooperates when needed if the facility faces an emergency or unusual circumstance.
  9. Participates in Continuous Quality Improvement activities through monitoring and evaluation of resident care.
  10. May be asked to participate on committees and community projects.
  11. Participate in staff meetings, in-service programs, performance improvement, and infection control activities as necessary.
  12. Promote teamwork with physicians, residents, families, visitors, and co-workers.
  13. Use proper body mechanics for transfers, lifting, etc…
  14. All other duties as assigned.

Full-time dayshift 7am-7pm weekdays/weekends
Hours vary, minimum of 36 hours
