QMHP-A or C for Mobile and Community Stabilization: This role requires work in the homes or community of adults and children providing at least 4 hours per day of community stabilization service. Part-time to Full-time hours = 28-40+ hours per week.

The QMHP is supervised by a Clinical Supervisor and corroborates with other staff members to ensure qualitative in-home/community services. A Qualified Mental Health Professional is expected to provide face-to-face services with the client in order to stabilize the individual primarily within the home, although they may occur in various community settings. The QMHP is required to assist in the development and implementation of the person-centered treatment and safety plan.


Duties and Responsibilities:

  • MUST HAVE COMPLETED MOBILE CRISIS RESPONSE TRAINING
  • Proper and timely documentation of all information and progress
  • Involvement in the preparation of individualized service plans and updates.
  • Provide various therapeutic interventions in home of Individuals experiencing a crisis.
  • Model and teach problem solving techniques.
  • Maintain continuous contact with the identified consumer and other key stakeholders (with signed authorization to release information) in planning and service delivery. These stakeholders might include, but are not limited to social workers, psychiatrists, court officials and police, teachers, and other human service professionals.
  • Respond to crisis(es) on 24 hours, seven days a week basis
  • Provide linkage to community-based services; provides advocacy and support for families with these services.
  • Provide education for parents, adult clients, and other supportive persons
  • Maintain client file in compliance and care coordination
  • Attend meetings, training and conferences as required.


Communication:

The employee will communicate effectively, in writing and orally, to caseworkers, individuals and family members, and staff of other service agencies. Employee must also maintain individual progress notes, discharge summaries, and additional documentation as needed.
