Responsibilities:

  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participates in the coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and Clinical Supervisor.
  • Uses and/or prepares equipment and supplies effectively and efficiently, adhering to aseptic technique.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.
  • An other job related tasks other than those stated in this description requested by the Clinical Supervisor.


Qualifications:

  • Valid nursing license (RN or LPN/LVN)
  • Minimum of 3 years of nursing experience, preferably in hospice or palliative care
  • Strong knowledge of anatomy, physiology, and medical terminology
  • Excellent communication and interpersonal skills
  • Ability to provide emotional support to patients and their families
  • Compassionate, empathetic, and patient-centered approach to care
  • Ability to work effectively as part of a multidisciplinary team
  • Strong organizational skills and attention to detail
  • Licensed driver with automobile that is in good working order and insured per the organization requirements.

Salary: $35.00 - $45.00 per hour
