Responsibilities:
- Provides direct patient care as defined in the state Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Participates in the coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and Clinical Supervisor.
- Uses and/or prepares equipment and supplies effectively and efficiently, adhering to aseptic technique.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
- An other job related tasks other than those stated in this description requested by the Clinical Supervisor.
Qualifications:
- Valid nursing license (RN or LPN/LVN)
- Minimum of 3 years of nursing experience, preferably in hospice or palliative care
- Strong knowledge of anatomy, physiology, and medical terminology
- Excellent communication and interpersonal skills
- Ability to provide emotional support to patients and their families
- Compassionate, empathetic, and patient-centered approach to care
- Ability to work effectively as part of a multidisciplinary team
- Strong organizational skills and attention to detail
- Licensed driver with automobile that is in good working order and insured per the organization requirements.
Job Types: Per diem, Part-time, Full-time
Salary: $35.00 - $45.00 per hour
