The community includes 84 units, a combination of one bedroom and studio apartments. 20 units are Memory Care and the rest are traditional Assisted Living. We have a bistro, a salon/spa, a fitness center and a rooftop terrace

Schedule:
- Per-Diem (All Shifts and Weekends)
- Per-Diem (All Shifts and Weekends)
- 16hrs/wk: 7a-3p: which includes every other weekend
- 24hrs/wk: 3p-11p: which includes every other weekend
- 24hrs/wk: 11p-7p:which includes every other weekend
Responsibilities:
- Assess and evaluate resident issues and medical concerns as needed and coordinates with other service providers to ensure proper care.
- Ensure exceptional quality of resident care is provided.
- Maintain accurate, detailed reports, data and records. Assist with all aspects of maintaining compliance with applicable state and federal guidelines.
- Accurately assess, observe, document and report any significant changes in resident’s behavior and health to the RN SALSA, and Executive Director.
- Serve as a liaison to families/designated representatives, health care agencies, emergency response teams, hospitals and rehabilitation centers.
- Direct, supervise, mentor, and guide the daily work assignments of the C.N.As.
- Collaborates with other team members to ensure quality of care and services to all residents.
- May perform other duties as needed and/or assigned.
Qualifications
- Current Connecticut LPN License
- Minimum of 1 year experience in a Assisted Living Community
- Flexibility with schedule
- Ability to work every other weekends
- Previous supervisory experience a plus
