Job DescriptionJob Description

This position is responsible for assessing the needs of individuals and/or their caregivers by providing information and referral services enabling them to make informed decisions based on available options.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  1. Conduct initial assessments, via phone or in-person, to determine client needs.

  1. Establish rapport with an inquirer and use active listening skills to determine the presenting problem.

  1. Assess client's current health condition, family supports in place and identify possible resource options.

  1. Provide eligibility screening to determine level of care criteria for public funded programs.

  1. Based on assessment, provides options to institutional care.

  1. Develop care plans for assigned clients and initiates services.

  1. Complete required client documentation according to program requirements. Records accurate transaction information for use in reports.

  1. Assess and monitors applicants on waiting list as assigned.

  1. Develop working relationships with area organizations, community resources and service providers. Makes contacts with medical offices and businesses as assigned.

  1. Conduct outreach activities for service referrals.

  1. Meet and maintain on an annual basis certification and training requirements of the FSSA Division on Aging.

  1. Travel locally as needed.

  1. Perform other duties as assigned.

Education and/or Experience

  • Continuous employment as a Care Manager by a AAA (Area Agency on Aging) since June of /2018; OR
  • One year of experience working with persons of intellectual or developmental disabilities (QMRP) or delivering health care/social services/care management and a bachelor’s degree in social work, human services, counseling, psychology, or other professional field of study; OR
  • A registered nurse; OR
  • An associate’s degree with one year of experience delivering health care/social services or care management, or at least two or more years in care planning, care management or delivering health care or social services; OR
  • Master’s degree in a related field
  • Degrees in social work or comparable fields preferred.
