Wellness nurses are responsible for conducting face to face wellness visits according to the service guidelines of the BDDS waiver-to include development, oversight, and maintenance of a wellness coordination plan. Care Coordination provision with the individual’s medical team and Sycamore Services’ employees. Ensure risk plans are created and implemented, when needed, then ensure staff are trained. Wellness nurses are active at team meetings and complete all documentation and reporting requirements. Other duties as assigned.
