Your Day-to-Day - Telehealth Coordinator

As our Telehealth Coordinator, you'll play a crucial role in managing the daily operations of our telehealth clients, ensuring seamless communication with physician providers and the interdisciplinary team. Your responsibilities will include coordinating changes in the plan of care, collaborating with local physicians and community providers, and supporting telehealth functions integral to our participation agreements and innovative programs. You'll have the opportunity to develop and refine telemonitor program policies and procedures, conduct video or telephone visits as necessary, and actively participate in quality improvement activities. Additionally, you'll oversee our Telehealth Technician, manage Telehealth inventory, and ensure the smooth operation of our telehealth services.

Requirements:

• Vermont Licensed LPN or RN with a minimum of five years of experience, preferably in cardiac telemetry, critical care, or emergency room departments
• Valid driver's license
• HIPAA-compliant space with high-speed, dependable internet access

Location & Details:

• This is a hybrid position, with 2-3 days expected in-office.
• Full-time, Monday-Friday, 8-5
