What We Are Looking For

The Behavioral Health Integrated Care Clinician will work as a Health and Wellness Consultant (HWC) within the Integrated Care setting providing Integrated Care services in departments as assigned. As a Health and Wellness Consultant (HWC), the BHC will work alongside primary care providers including physicians, nurse practitioners, physician assistants, and other healthcare staff to deliver behavioral health Integrated Care services including brief psychotherapy in the medical clinics. In this setting, the BHC will focus on psychosocial stressors and mental health symptoms that may impact coping with chronic disease, compliance with medical treatment, and engaging in preventive health behaviors. BHC staff will develop treatment plans integrated with the patient's medical treatment plan and will work closely with the medical team to meet the needs of individual patients. The BHC will participate in weekly clinical supervision with licensed staff with the expectation of completing clinical hours for licensure within three years.

Schedule: Per Diem, as needed, up to 10 hours per week - This is a Non-Benefited Position | Location: San Bernardino, CA

ESSENTIAL FUNCTIONS AND DELIVERABLES

  • Ability to use evidence-based behavioral health assessments and interventions as part of the Integrated Care team for patients in the medical clinics with chronic health needs, mental health needs, or who may be at risk for mental health related issues. Uses motivational enhancement strategies to improve patient engagement in healthcare treatment plan.
  • Provides culturally sensitive psychoeducation to ensure fair and equal care to all clients in the development of appropriate service plans and referral and linkages to internal and external resources based on individual/family needs.
  • Provides family with necessary case management information, facilitates referrals to community resources, serves as liaison among clients, integrative health care staff, and community agencies.
  • Attends and participates in monthly staff meetings including the monthly HWC meeting, monthly HBWC clinical staff meetings, monthly Provider meetings and SACH General Staff Meetings, in-services, staff development and participates in educational courses, workshops and conferences.
  • Provides behavioral health related crisis intervention services as needed.
  • Conducts regular follow-up assessments to monitor patients' progress and adjust treatment plans accordingly, ensuring continuity of care and promoting positive outcomes.
  • Facilitates support groups for patients with similar mental health concerns, fostering a sense of community and peer support within the clinic setting.
  • Participates in interdisciplinary case reviews and treatment planning meetings to address complex patient cases and identify opportunities for collaborative care coordination.
  • Conducts outreach and engagement activities within the community to raise awareness of behavioral health services available through the clinic and reduce stigma associated with mental illness.
  • Engages in research activities or quality improvement projects related to integrated behavioral health care, contributing to evidence-based practices and continuous improvement efforts within the organization.
  • Utilizes electronic health record systems effectively to document patient assessments, treatment plans, interventions, and progress notes in accordance with organizational and regulatory guidelines.
  • Utilizes telehealth platforms to expand access to behavioral health services for patients in underserved or remote areas, ensuring equitable access to care.
  • Adheres to the NASW or APA Code of Ethics as outlined within discipline scope.
  • May mentor graduate students/interns as assigned.

QUALIFICATIONS:

  • Education: Must possess a Master's Degree in Social Work (MSW) or Post-Doc Psychologist or closely related field.
  • Licensure/Certification: Must be eligible for LCSW licensure or PhD of PsyD within three (3) years of date of hire. A current CPR/BLS certification (must be American Heart Association or Red Cross accredited program). As a requirement of this position, you must receive EPIC certification for the module you have been hired into. Valid California driver's license, and auto insurance.
  • Experience: Minimum of 2-3 years of clinical experience required. Community clinic experience preferred. Experience with substance use treatment preferred. Multidisciplinary experience within the medical field preferred. Experience providing psychotherapy in Spanish preferred. Trauma-informed clinical orientation and experience preferred.
  • Essential Technical/Motor Skills: The ability to use a computer on a routine basis and complete required documentation. Use of word processor, spread sheets and internet. Ability to transition to Electronic Health Records.
  • Interpersonal Skills: The ability to communicate positively and effectively with all staff in a multidisciplinary environment. Ability to conduct meetings and present in a public forum. Strong interpersonal relations skills. Fluency in English and Spanish preferred. Ability to maintain a professional attitude in difficult situations and communicate effectively. Understands needs of clinic and works well in a team environment.
  • Essential Mental Abilities: Must have critical thinking for comprehensive treatment planning, empathy for building therapeutic rapport, cultural competence for engaging diverse populations, emotional intelligence for effective communication, adaptability for navigating complex environments, and strong communication skills for collaboration with interdisciplinary teams. Together, these abilities empower clinicians to provide compassionate, culturally responsive, and evidence-based care, promoting holistic well-being and positive patient outcomes.
