DUTIES AND RESPONSIBILITIES:

  1. Must possess the ability to plan, develop, organize, evaluate, and implement the programs, goals, objectives, policies and procedures, etc., of the physical therapy program to hlp residents obtain/maintain their highest level of physical, mental and social well-being.
  2. Interpret and/or instruct in physical therapy policies and procedure to personnel, residents, family members, etc. as necessary.
  3. Participate in discharge planning, development and implementation of resident care plans, resident assessments (MDS), etc. as necessary.
  4. Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
  5. Maintain treatment records, resident files, and progress notes as required
  6. Maintain an adequate liaison with families and residents.
  7. Perform any other duties as deemed necessary and appropriate, or as may be directed by the Director of Rehabilitation and/or the Administrator.
  8. Maintain satisfactory attendance.
  9. Arrive and be ready to work at designated times.
  10. Ensure that the therapy area is maintained in a clean and safe manner for resident comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.
  11. Assist to develop and participate in programs designed for in-service education, on the job training and orientation classes for newly assigned personnel.
  12. Participate and assist in departmental studies and projects as assigned or that may become necessary.
  13. Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, support agencies, and outside agencies.
  14. Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  15. Ensure that established policies and procedures of the facility are followed at all times, including infection control procedures and safety procedures.
  16. Ensure that residents' rights are established and maintained at all times, including confidentiality, privacy, refusal of treatment, and their right to participate in the planning and scheduling of his/her treatment.
  17. Works beyond normal working hours and on weekends and holidays when necessary.
  18. Attend and participates in continuing education programs as may be required by current regulation.
  19. Is subject to injury from falls, burns from equipment, odors, and exposure to infectious condition
  20. Is subject to sitting, walking, bending, stooping, pushing, pulling, as well as lifting, carrying and supporting residents. Must be
