The Clinician consistently exercises discretion and judgment to analyze, interpret, make deductions, and then decide

what actions are necessary based on the varying facts and circumstances of each individual case. The Clinician

provides direct treatment services to youth and families placed at Thrive & Succeed Group Home to include individual, group,

family, and when needed, case management services. The Clinician ensures that all documentation – including

treatment session notes and monthly summaries are completed in a timely fashion.

Requirements:

Serves as a member of Thrive & Succeed Group Home Services’ interdisciplinary team.

• Ensures that all activities, services, and programs through Thrive & Succeed Group Home are conducted in a manner

that is sensitive to and shows respect for the cultural and ethnic diversity for all Thrive Group Home clients

• Develops, implements, and revises individual treatment plans; assures that services provided are specified in

the treatment plan and monitors progress toward treatment goals.

• Provides face to face crisis management services for clients, including assessing the crisis and collaborating

with others.

• Provides individual and group skills training and development to assist the client and/or family in developing

skills (i.e. self-care, daily life management, interpersonal communication, parenting, illness management,

anger management, positive coping skills, and problem-solving skills) directed toward reducing psychosocial

barriers.

• Works collaboratively with the medical staff to ensure coordination of integrated care for the client, including

identification of health issues, compliance with treatment, and effective client to medical provider

communication.

• Assists in the completion of the CANS re-assessments.

• Provides consultation to staff on specific cases involving coordination of care.

• Maintains positive working relationships with staff of other agencies and institutions, homes, and facilities,

and acts as a liaison between clients and agencies and/or institutions.

• Prepares and maintains clinical records, reports and forms and other administrative tasks as required.

• Participates in continuing education activities, remaining knowledgeable in areas of expertise.

• Maintains neat, chronological clinical documentation of all sessions, contacts, and casework with clients,

families, and community resources.

• Ensures that consistent behavioral-based treatment occurs by providing guidance and training to employees

for assigned clients.

• Interacts professionally with parents, school, medical personnel, and others using only excellent interpersonal

and customer service skills.

• Keep abreast of current treatment technology, literature, practices, and regulations.

• Attends meetings, conferences, and workshops as recommended by the Assistant Director Behavior Support

Services.

• Will adhere to all policies and procedures as defined in the Employee Handbook and Thrive & Succeed Group Home

Operational Policies and Procedures Manual.

• Takes pride and ownership in Thrive Group Home’s facilities and assigned work areas.

• Attends mandatory staff meeting and other meetings as assigned.

• Completes and updates mandatory training and other licensure related items in a timely manner.
