Job Description:
The Client Care Coordinator will provide recovery focused services related to the safe and appropriate administration of medical treatment (including medications) as prescribed by the physician. Client Care Coordinator will be responsible to provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care. Shifts available: 8:00 am – 8:30 pm, 8:00 pm – 8:30 am
Job Duties:
- Ability to maintain confidentiality of patients' medical and personal information
- Respond to emergencies and initiate action as required
- Observation, documentation, and record keeping of client’s health status
- Be familiar with common detox assessments, CIWA and COWS
- Demonstrates knowledge of crisis management, common interventions & skilled in time management
- Ensures client compliance with facility rules and guidelines
- Follows HIPPA standards/laws in all aspects of their duties
- Provide care and assist in the welcoming, admission and discharge process for clients
- Collects data relevant to person served care
- Develops and implements nursing care plans
- Transcribes physician orders accurately and in a timely manner
- Collect, read and document Urine Analysis
- Observes client self-administer medications, as prescribed
- Receives and presents change of shift report, as assigned
- Turn over bedding, as needed
- Adheres to all policies and procedures
- Assists with conducting TB (if capable), Pharmacy, and store runs
- Notifies the prescriber and Client Care Supervisor of any changes in behavior and/or condition on a real time basis
- Monitor and itemize OTC medications/supplies and notify management of all medications/supplies to be ordered.
- Sanitizing all stations and areas of the facility
- During downtime will assist with household chores and help with the overall functioning of the facility (general cleanliness and safety of the house) which includes but is not limited to laundry (washing, folding, putting away), bathroom inventory (stocking toilet paper, paper towels, wiping down counters, empty trash, cleaning mirrors), food handling (daily check of any expired food items from the fridge & ensuring that all food items are labeled with date of expiration), and maintain cleanliness of rooms and common areas throughout the facility (sweeping and mopping).
Minimum Requirements and Qualifications:
- Current LVN, MA, EMT, or Paramedic license in the State of California (required)
- Current CPR/First Aid (required)
- Minimum 1 year of clinical nursing experience
- Minimum of 1 years’ experience in chemical dependency (required)
- Proficient knowledge of chemical dependency and psychological disorders
- Ability to communicate and work effectively with diversity (required)
- Knowledge of detoxification protocol and related care including symptoms and medications
- Ability to accurately record information and prepare reports
- Strong ability to establish and maintain effective working relationships with the staff, patients, patient's families and authorized consents
