The Hospice Nurse Residency Program is a 3 month immersion orientation focused on the conceptual and practical component of Hospice Nursing. The program is ideal for newly licensed registered nurses or registered nurses with less than 1 year nursing experience.
Under the supervision of a registered professional nurse, the Nurse Resident will assist with organizing and administering individualized, professional patient care and psychosocial/ spiritual interventions for assigned patients/families throughout their length of stay in various locations. Under the supervision of a registered professional nurse, the Nurse Resident will assist with providing nursing care in accordance with the objectives, policies and procedures of the organization and the state Nurse Practice Act.
Essential Duties and Responsibilities for a Nurse Resident:
- A Nurse Resident demonstrates ability to perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient's well being, comfort, and dignity throughout the dying process and documents findings on all appropriate forms and on the plan of care (PIO).
- Performs initial pain assessment using the Edmonton scale, adhering to pain protocols, and knowledge of equianalgesic dosing and assures optimal pain relief using an interdisciplinary focus and time appropriate interventions.
- Able to articulate clinical course/disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, neurologic disease states.
- Able to articulate etiologies of symptom manifestation (nausea, vomiting, anorexia, fluid and electrolyte imbalance, diarrhea, constipation, mucositis, weakness/fatigue, dysphasia, edema, dyspnea, cognitive impairment, urinary retention, depression, delirium) related to a disease process.
- Modifies the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control.
- Demonstrates knowledge in usage, indication, adverse side effects, contraindications and negative interactions of most commonly used steroids, NSAIDs, benzodiazepines, antidepressants, anticonvulsants, narcotics, antifungals, diuretics, etc.
- Able to articulate Hospice philosophy, the meaning of palliative care, and end of life goals of care (safe and comfortable dying, self-determined life closure, and effective grieving).
- Organizes and prioritizes assignments to consistently meet productivity standards and complete documentation requirements within identified time parameters
- Effectively executes patient/family/caregiver/facility staff teaching in all given situations
- Able to articulate understanding of the four levels of Hospice care (routine home care, continuous care, Inpatient care, respite care) and initiates appropriately.
Qualifications for a Nurse Resident:
- Education / Certification/Experience
The Nurse Resident candidate will be a graduate of an accredited school of nursing and licensed to practice in New York. Previous work experience, preferably in health care. A commitment to the professional standards of nursing, excellent communication skills and a high level of enthusiasm with motivation to succeed. Current NYS drivers license and a car available for use when on duty.
