The Acute Stabilization Unit (ASU) is a short-term, voluntary inpatient treatment setting for people experiencing crises of mental health and substance use disorders. The ASU provides stabilization and referral to treatment, and the case manager is responsible for discharge planning and coordination of care. At the ASU, the case manager coordinates discharge planning, serves as a liaison to community mental health centers and other partner organizations, and transitions identified clients to appropriate after-care services.
The case manager is proactively in identifying needs and in finding solutions, resources, and strategies to meet these needs, working to help clients identify their challenges, resources, strengths, supports, and goals. To help clients meet their goals, the case manager works not only with clients, but with the ASU treatment team, the client’s supports, and external agencies. An effective case manager maintains relationships with community providers and advocates for clients’ right to access quality care.
The case manager is responsible for coordinating with a client’s existing providers about admission and discharge appointments. The case manager maintains communication with residential treatment providers and refers clients to ongoing treatment in the community, including but not limited to psychiatry, therapy, primary care and other medical providers, substance use treatment, and other supportive services as needed.
The idea candidate has an understanding of mental health and substance use disorders, and familiarity with challenges and resources relevant to people in early recovery. The case manager must be a strong advocate with energy and creativity, who is able to foster productive relationships with clients and with community providers.
