QUALIFICATIONS
In order to fulfill the job functions listed below an individual must meet the following minimum qualifications:
Communicate effectively in person, on the telephone, and in writing.
Possess current licensure as a Registered Nurse in the State of Michigan.
Experience in performing the assessment process (Preferred).
Experience in computers (Preferred).
Minimum of one-year experience in geriatrics (Preferred).
Minimum of one-year experience in a supervisory capacity (Preferred).
Experience in MDS (Preferred).

Comply with all safety, universal precautions and infection control policies and procedures.
Be responsible and supervise the safety of residents in the Facility.
Present a friendly, calm, cooperative, positive manner.
Arrive to work as scheduled and consistently demonstrate dependability and punctuality.
Obtain continuing education relating to the care of the geriatric resident and nursing education.
Maintain licensure as a Registered Nurse in the State of Michigan.
Assume a position on Nursing Management On-Call Schedule Rotation.

On occasion may need to lift up to 40 lbs.
Direct, evaluate, and supervise all resident care and initiate corrective action as necessary.
Review residents admitted and compare physician orders with each resident’s plan of care to assure
appropriate implementation of the physician’s plan of care.
Assist in the admission, transfer, and discharge of residents and to complete admission, transfer, and
discharge documentation.

Prepare MDS required for reimbursement purposes to include tracking, completion, and timely submission
of these documents.
Make recommendation on documentation in the nursing record of residents, as necessary.
Assist the Director of Nursing in the investigation process for suspected abuse or neglect.
Assist/support the Director of Nursing with the counseling of employees.
Listen to resident, family, or physician complaints and initiate corrective and/or report special problems to
the Director of Nursing.
Assist with the hiring process as requested.
Conduct and participate in resident care plan Team Conference. Conduct Team Conferences weekly.
Gather all information contained in Team Conference summaries and ensure data is documented.
Support the Quality Assessment and Assurance Committee with the data collection and reports.
Serve as a member of the QAPI.
Gather data for initial MDS as well as, change of conditions, quarterly, and annual reviews on all residents,
and obtain signatures.
Review reports of timely completion of residents’ MDS assessments and care plans with follow-up action
as necessary.
Schedule all resident MDS’s and Team Conferences in accordance with state and federal guidelines.
Enter nursing related problems onto computer for care plans.
Gather information for MDS, enter into computer on due date, then complete corresponding CAA analysis
with Triggers and Potential Triggers.
Complete reports and assignments accurately and adhere to established time schedules.
Render immediate assistance for all residents’ welfare as necessary.
Identify and observe safety hazards and initiate corrective action as necessary.
Work cooperatively with administration, all nursing service personnel, physicians, community agencies,
residents, families, and consultants.
Establish open lines of communication between all nursing personnel and take follow-up action as
necessary.
Observe, interpret, and implement established Facility policies, procedures, and regulations to RN’s, LPN’s,
and CNA’s.
Supervise and evaluate LPN’s, RN’s, and CNA’s conformance to Facility policies, procedures, and
regulations.
Assist the Director of Nursing to update or revise procedures as needed, offer input and recommendations
in the formation of new nursing procedures.
Observe, follow, supervise, and evaluate the implementation of resident rights at all times.
Observe the Facility safety rules to nursing staff.
Assess resident equipment and supply needs and make recommendations to the Director of Nursing.
Participate in Facility educational programs including, but not limited to, in-service for Facility personnel.
Evaluate resident and staff needs and provide the Administrator and the Director of Nursing with
comprehensive recommendations concerning the educational needs of nursing assistants.
Assist with the infection control program as requested.
Interview, counsel nursing staff and recommend personnel or disciplinary action to the Director of Nursing
as necessary.
