What you’ll do:

As a member of the clinical team, the Employment/Rehabilitation Specialist FEP completes assessment and service plan and implements services to consumers in accordance with their individualized service plan (ISP). Communicates and documents consumer’s progress toward their recovery. Coordinates and monitors rehabilitation-related services for consumers with serious mental illness. Works with consumers who have been identified by the clinical team, utilizing the criteria set forth in implementing the IPRPT Boston University Model. Works in partnership with assigned RSA Vocational Rehabilitation Counselor, involved to assist consumers increase or maintain involvement in learning, working, living and social activities.

Essential Responsibilities:

  • Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with consumer, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented.
  • Deliver 40 hours of work per week, with a goal of 30 hours direct client contact, which includes case management as needed. Productivity standards established at a minimum of once a year and as needed due to contract changes. Evening and weekend hours may be required to accommodate the schedules of individuals served.
  • Attend meetings and trainings as requested.
  • Attend regular scheduled weekly supervision. Seek out additional supervision as needed.
  • Utilize the Boston University Intensive Psychiatric Rehabilitation Program Team Approach (IPRPT) to assess consumer’s needs including living, learning, working, and social leisure goals and assist consumers on assigned caseloads to reach their desired role achievement in any or all of these environments.
  • Provides supportive services including, but not limited to, the following:
  • Assistance in maintaining, monitoring and modifying covered behavioral health services;
  • Brief telephone or face to face interactions with a person, family or other involved party for the purpose of maintaining or enhancing a person’s functioning;
  • Serves as a point of contact and to ensure ongoing collaboration including the communication of appropriate clinical information with other involved parties as appropriate and coordination of care with a person’s family, behavioral and general medical and dental health care providers, community resources, and other involved supports including educational, social, judicial, community and other State agencies.
  • Participates in case conferences or other meetings with or without the person or his/her family participating.
  • Screens and assesses all persons on caseload for financial entitlements (AHCCCS, SSI/SSD etc.); completes AHCCCS applications on all consumers on caseload meeting criteria.
  • Develops relationships with community employers, providers, or other organizations to foster an increase in employment and meaningful community activity opportunities for consumers.
  • Conducts an “Orientation to Rehabilitation Services” monthly.
  • Educates team members as the subject matter expert regarding rehabilitation and vocationally related issues and provides ongoing technical training and support to the team as requested or indicated including updates on available community resources, how to utilize the services of the co-located RSA/VR counselor or co-located vocational providers.

  • Must have a valid AZ driver's license with no major infractions in the past three years.
  • Must have a Fingerprint clearance card or be eligible to obtain one.
  • Must be at least 21 years of age at the time of employment.


Education and Experience:

  • Bachelor’s degree in Psychology, Counseling, Social Work, or related field preferred, Or, Associate degree and a minimum of two years full-time behavioral health work experience. Or, High School Diploma/GED with four years of full-time behavioral health work experience

  • Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender or sexual orientation.
  • Maintain professional conduct consistent with ethical professional practice and the policies and procedures of the agency.
  • Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners.
  • If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.

  • The typical work environment will be working indoors, in an office environment, or working in the community and in a members’ home.
  • Position requires driving of company van and/or personal vehicle.
  • Must maintain a valid Arizona driver's license.
  • Must maintain liability insurance for personal vehicle.
  • Employees must be able to lift up to 15 pounds at times.
