Seeking Motivated and Compassionate Home Care Nurse

Caring Hands United, Inc is seeking a LPN for home care supervisory visits, client admission assessments and coordination of homecare services in the Soperton and Mount Vernon, Ga. Must be able to commit to at least 12 home visits in a two (2) week period, have reliable transportation and able to travel within a 50 mile radius.
DUTIES
A) Works to promote maintain and restore healthy living in the community for homecare clients and confer with RN regarding changes in the Plan of Care.
B) Instructs and supervise through the educating of the home health aide, clients and family members, the prevention of infection, accident, and injury.
C) Completes an initial Admission Assessment and on-going supervisory visits to monitor the Caring Hands United, Inc services.
D) Assessment of the client's needs/problems at appropriate time points.
QUALIFICATIONS
? Current and unrestricted Georgia LPN licensure
? A minimum of one year nursing experience preferred
? Strong medical surgical, geriatric care experience preferred
? Home Health experience a plus
? Current CPR certification
? Good organizational and communication skills
? Available to travel within Laurens and surrounding counties.
