Job Title: Medical Case Manager

Location: Augusta , ME

Duties:

  • Shall track medical status of all assigned Soldiers.
  • Schedule profile exams/evaluations and audiology exams at periodic health exam facility.
  • Coordinate Soldier treatment plans with appropriate medical facilities.
  • Correspond with soldiers to obtain information for the Soldier’s case management file, referred by a provider are followed.
  • Manage Soldier’s care from the identification of the condition to when the Soldier is either considered medically ready and returned to duty or administratively separated from the military.
  • Educate Soldiers on resources available to improve medical and dental readiness.
  • Review and track health assessment lab results and contact SM and educate on lifestyle modifications.
  • Update MODS as required on the following: immunizations, designation of pregnancy, eye exams, and medical warning tags.
  • Track medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentation.
  • Shall prepare medical documentation for Line of Duty (LOD) investigation for units for which they are responsible in conjunction with Health Service Support (HSS).
  • Shall mail all medical records to ensure timely and accurate processing of medical records to and from mobilization sites, 30 calendar days prior to soldiers’ report date.
  • Shall enter individual vouchers for SM IMR requirements into the Automated Voucher System (AVS) when a soldier misses a MRE or SRP.

Qualifications:

  • Shall have at least 5 years of case management experience
  • Minimum of 3 years clinical experience.

Education/Certification:

  • Registered Nurse III (RN) License
  • Preferred Certified Case Manager (CCM) Certification.
