Under the supervision of the Director of Inpatient Units, the Unit Manger is responsible for the Inpatient Units (IPU) (Martin and Saint Lucie Counties locations) supervision, planning and implementation of delivery of direct clinical services to patients and families in the interdisciplinary setting that identifies hospice end of life care. Also responsible for coordinating the interdisciplinary team services, and the effective and efficient use of resources.

Qualifications

  • Three (3) years of hospice and/or home health experience within the past five years and one year of managerial experience required.
  • Must be Certified Hospice and Palliative Registered Nurse (CHPRN) within 12 months.
  • Current Florida license as a Registered Nurse.
  • Knowledge of Hospice Conditions of Participation (COPs) required.
  • Must have the demonstrated ability to execute an effective, supportive, sensitive approach in response to the needs of patients, families, and staff.
  • Demonstrate skills in administrative performance, including expertise in both written and oral communication is required.
  • Must have strong organizational and supervisory skills with the ability to multi-task.
  • Knowledge of documentation standards, service coordination, and case management.
  • Maintain a high level of urgency, customer consciousness and service excellence.

EXCEPTIONS:

Provide program development strategies for Treasure Health, in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Inspire staff to live the Treasure Health’s Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.

Key Result Areas

  • Educates, mentor and coach staff to ensure excellent customer service, high level staff performance and clinical excellence as measured by positive feedback from the Family Satisfaction Survey, QI team performance, integrity of staff documentation.
  • Teach and uphold Treasure Health’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress & behavior.
  • Educate and teach employees Treasure Health’s Standards.
  • Lead, develop and motivate employees with confidence, energy, integrity and resources.
  • Responsible for the management and coordination of the day to day operations of patient care within the Inpatient Units. Outcome measured of employee/client feedback, family outcomes/feedback, and employee performance levels. Works collaboratively with other Team Managers and Directors to accomplish this goal.
  • Participate and provide leadership during the regular interdisciplinary team meetings, with oversight of the development, review and revision of the patient/family plan of care.
  • Work with team to facilitate well organized, productive and timely IDT meetings.
  • Responsible for the assignments of appropriate staff to provide care to patients and families, utilizing the appropriate staffing standards and working in conjunction with other Team Managers to assure best assignments based on patient/family needs.
  • Coordinate the effective use of DME and Pharmaceuticals.
  • Accountable for the oversight and accuracy of staffs’ payroll.
  • Promote consistency on the team in the area of accountability, adherence to policy and procedures, punctuality and absenteeism.
  • Responsible for rotation Administrator duties.
  • Capacity to provide oversight and primary patient care duties- as needed.
  • Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each.
  • Participate as an active supporter of the Treasure Health compliance program including the emphasis to report, as well as complying with all Federal, State, and Local laws as well as the organization’s compliance program.
  • Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.

OSHA Category

Position will be categorized as category III (involves no routine or potential exposure to blood, body fluids/tissues).

Working Conditions

  • Typing or otherwise working primarily with fingers rather than with the whole hand as in handling.
  • Sedentary – Exerting up to ten pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria met.
  • The work is required to have close visual acuity such as/preparing and analyzing data and figures; transcribing, viewing a computer terminal; extensive reading.
  • Lifting – Frequent lifting up to 20 pounds.
