The mission of Grant a Gift Autism Foundation (GGAF) – Ackerman Center is to help children, young adults, and their families master living with autism by providing access to the best-in-class diagnostic and treatment services, vocational programs, family support, and case management to all families facing the challenges of autism and other neurodevelopmental conditions.

The Family Service Department’s primary role is to develop strong relationships with families in order to provide service coordination, support, and training to assist families in meeting the needs of their children. They serve as the primary point of contact of family members, ensuring they maintain an active line of communication.

Job Description

The Care Coordinator will provide quality case management for families as they navigate the systems necessary to support their child with autism or other neurodevelopmental conditions. The Care Coordinator is responsible for guiding families through programs and services, consulting with families and determining their needs, coordinating with the clinical teams on care plans, educating them about autism and other neurodevelopmental conditions, and working with the care team to evaluate interventions. The Care Coordinator will provide onsite case management, customer service and support.


Duties and Responsibilities

  • Support families that contact Grant a Gift Autism Foundation Ackerman Center for services, questions and referrals.
  • Supports care team members by providing comprehensive case management support to patients and their families.
  • Coordinate the transition of patients/families through various service lines and programs.
  • Develop a rapport with families by facilitating thorough communication from initial contact to post-service follow-up.
  • Accurately document patient/family needs and services provided within an Electronic Medical Record (EMR), to ensure care team members can track on-going care.
  • Attend ongoing training and courses to keep abreast of new developments in healthcare, within the disability field.
  • Treat patients with empathy and respect; conduct oneself in a professional manner.
  • Comply with organizational guidelines and health care law and regulations.
  • Identify and follow-up on all referrals made to assure continuity of care and patient/family needs are met.
  • Communication with providers and care team regarding patient progress and care needs.
  • Assist with referral process from and to community agencies through the development of relationships and partnerships.
  • Research, attend and participate in local resource fairs.
  • Assist with the facilitation of program orientations, parental training(s), sibling and teen groups, and other family related workshops and training.
  • Support the cultivation of community partnerships for outings, case management outreach, referrals, and family training.
Requirements:

Educational Requirements

  • Bachelor's degree required.
  • Spanish bilingual a must.


Qualifications

  • Passion for the mission and the work the organization does for children, teens, young adults and families affected by autism and other neurodevelopmental conditions.
  • At least 1 year of experience working with low income and/or vulnerable populations – does not have to be autism/neurodevelopmental conditions related.
  • At least 1 year of previous applicant eligibility and intake experience.
  • Ability to discuss sensitive issues with families using compassion and empathy.
  • Experience and working knowledge of healthcare regulations, and federal support systems (e.g., Medicaid, Medicare, SSI, and other government and commercial insurances).
  • Experience working within a non-profit, clinical training center (Preferred)
  • Strong working knowledge of Microsoft Office, especially Excel, Word, and Outlook.
  • Excellent written and oral communications skills.
  • Creative, professional, and resourceful with the ability to work independently and a part of a multidisciplinary team.
  • Ability to take initiative and complete multiple tasks and projects on time.
  • Previous experience working within an Electronic Medical Record (EMR).
  • Detail orientated and strong organizational skills.
  • Bilingual in English and Spanish is required.
