 Job purpose                                                                                                                                                                                 

The Child and Adolescent Clinician IOP will provide Intensive outpatient clinical services to children at Shoreline Family Health Care (SFHC). Additionally, the Child and Adolescent Clinician IOP will Provide group and individual therapy in both IOP and OP settings.  The Clinician support the goals FHCHC by providing clinical services based on best practices to individuals in the outpatient clinic while collaborating with psychiatrist and treatment team in diagnosis and treatment plan. Services will be provided under the supervision of the Director of Behavioral Health  /or their designee.

Duties and Responsibilities                                                                                                                                                                                                     

  • Provide structured, intensive group therapy in accordance with program policy to assigned caseload of children
  • Provide clinical consultation to schools and various community resources, as well as to FHCHC staff and other programs in the Center
  • Maintain regular collateral contact with parents, schools and other programs
  • Evaluate and treat individuals with psychological and/or substance use disorders within a Primary Health Care setting and/or community mental health setting.
  • Assess clients’ abilities and mental health status, needs and preferences and ongoing appropriateness of services.
  • Conduct intake assessments and initial treatment plans.
  • Provide group and individual therapy in both IOP and OP settings
  • Assist clients in caseload with developing and implementing individual treatment plans. May coordinate comprehensive (integrated) service planning to include members of clients’ treatment team and clients’ natural supports, including family members, etc. Conduct regular reviews of treatment plans and modify as needed.
  • Provide emergency assessment and crisis intervention as needed. Communicate client needs with hospital emergency rooms and emergency personnel.
  • Accept warm handoffs at SFHC in collaboration with primary care providers following the integrated primary care model.
  • Monitor and analyze utilization review and productivity of caseload and make adjustments as needed.
  • Complete progress notes documenting progress on service goals and objectives, psychosocial histories, admission and discharge summaries, and other documentation within agency guidelines for timeliness in the electronic health record identified by location.
  • Obtain insurance authorization for clients as required.
  • Provide timely documentation and data to agency departments, DMHAS, or other sources as requested.
  • Perform supervised urine screening and Breathalyzer test as needed
  • Oversee program discharge planning for assigned caseload.
  • Collaborate with hospital staff regarding inpatient treatment and discharge planning.
  • Serve as a liaison to other Agency programs, other parts of the mental health system, and 2 other community resources. Provide advocacy as appropriate.
  • Inform and refer clients for benefits for which they may be eligible. Complete clinical documentation for benefits as needed.
  • Participate in program planning and development.
  • Provide backup for clients’ needs as necessary in the absence of other clinicians.
  • May be required to use own vehicle on company business.
  • May be required to travel between clinic locations during business day to meet client or supervisor needs.
  • Attend training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
  • Perform other duties as assigned.
  • Regular and consistent attendance is an essential function of this position                                                       

Qualifications                                                                                                                                                    

 Master’s degree in social work or related mental health field required. Minimum of two years of clinical experience in a psychiatric or substance abuse setting working with adolescents and children preferred. Must be proficient with PCs and Microsoft Office Word & Outlook, and if applicable ability to navigate through electronic health record.

License- required. Professional CT Licensure: LCSW;, LPC, LMFT or licensed psychologist considered. Bilingual English/Spanish highly preferred.
