We are a Support Services Brokerage and a nonprofit 501(c)3 agency that provides case management to adults with intellectual and developmental disabilities living in the communities of Coos, Curry, Douglas, and Klamath counties.
Position Available
We are seeking a full-time Personal Agent (Case Manager) to work in our Roseburg office.
This is a 40-hour-per-week position, to be worked within the core organizational hours of 8:00am – 4:30pm, Monday through Friday, with periodic flexibility to accommodate customer or organizational needs as required.
This is not a direct support position, but rather a comprehensive case management position.
As a Personal Agent, you will help customers (the individuals we serve) and their families navigate services. Through a person-centered process, you will meet with customers and their chosen participants to plan, develop, and implement individualized support plans (ISPs) based on their needs, goals, and dreams. You will advocate with and on behalf of your customers to obtain needed services, resources, exceptions, etc. You will provide choice advising and facilitate connection with providers. You will also monitor the quality of services provided and attend to changes in need and emergent issues as they arise.
To be successful as a Personal Agent, you must possess the following skills/abilities:
- Strong computer skills including but not limited to experience with MS Word, Outlook, and some Excel.
- Strong documentation/written communication skills: Documentation is a critical function of this job. You must be able to produce and maintain accurate and robust documentation.
- Excellent verbal communication skills: You must be able to communicate effectively with a variety of people, agencies, programs, etc.
- Interpersonal skills/relationship building: The ability to get along well with others and build authentic rapport is extremely important in this position. This includes but is not limited to our customers, their families, community partners, agencies, and fellow Connections staff.
- Strong organizational and problem-solving skills: You must be able to work independently and efficiently to organize your time, manage your workload, and meet deadlines.
Minimum Qualifications
- A bachelor's degree in behavioral science, social science, or a closely related field; or
- a bachelor’s degree in any field and one year of human services related experience; or
- an associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience; or
- three years of human services related experience.
- Oregon Class C driver’s license with an acceptable driving record and valid insurance.
- Ability to pass a background check.
- Residence in Oregon AND within the county/counties served (or reasonable distance).
Qualifications that will set you apart from other applicants
- At least 6 months of case management experience.
- Knowledge of the public service system for developmental disabilities services in Oregon.

Compensation & Benefits
Compensation is $31 per hour to start with an increase to $32 after successful completion of orientation.
