General Statement:
Under the direction of the Program Supervisor, facilitates the development of individualized Wraparound Plans with families of children with Serious Emotional Disturbance (SED) through the Wraparound process. Assists families in identifying and accessing resources, clinical treatments, and other services to promote the child/youth functioning successfully in the home, school and community; performs services as part of a Child and Family Team and in coordination with a wide variety of agencies and community supports including DHS, Family Court, and the schools; Performs services primarily in the home and community; performs related work as required.

Minimum Requirements:

Bachelor’s Degree in Social Work and Michigan Level Social Worker (LBSW) credential, Clinical AND one (1) year of experience working with children, adolescents and their families in a clinical setting. Will consider a candidate with a Limited License as a Bachelor’s Level Social Worker (LLBSW) credential, Clinical AND one (1) year of experience working with children, adolescents and their families in a clinical setting, OR a candidate with a Bachelor’s degree in a human service field (defined as Counseling/Guidance or Psychology), who also must possess a Bachelor Level Social Worker (LBSW) credential AND one (1) year experience working with children, adolescents and their families in a clinical setting. The employee with a LLBSW must take and pass the ASWB Examination within four (4) years of hire to retain employment. The employee with a LLBSW must also make application for the LBSW upon completion of 4000 hours in order to retain employment.

Employee must maintain valid licensure to retain employment.

Additional Requirements:
Must retain DCH qualifications as a Child Mental Health Professional.
Must complete DCH Wraparound Training.
Must have a valid Michigan driver’s license, current automobile insurance, and the use of a vehicle during work hours.
Complete and pass CAFAS/PECFAS training sustaining reliable rater status for assessment instruments.
Ability to physically perform the tasks.

Veterans must submit Form DD214 to determine eligibility for Veterans’ Preference
