Job Description:
We're seeking an Experienced Home Health Nurse with a minimum of 1 year of home health nursing experience. Proficiency in Axxess software and top-notch charting skills are a must. You'll play a crucial role in delivering high-quality care and maintaining accurate documentation.

Key Responsibilities:

  • Conduct thorough health assessments in clients' homes.
  • Develop personalized care plans for optimal outcomes.
  • Administer medications and skilled nursing care.
  • Monitor patient progress and adjust care plans as needed.
  • Educate patients and caregivers.
  • Maintain precise electronic records using Axxess software.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Ensure compliance with home health regulations.

Qualifications:

  • Minimum 1 year of home health nursing experience.
  • Proficiency in Axxess software with at least one year of hands-on experience.
  • Strong charting and documentation skills.
  • Current nursing license in Illinois.
  • Excellent communication and interpersonal skills.
  • Compassionate and dedicated to high-quality care.
  • Ability to work independently and collaboratively.
  • Reliable transportation and valid driver's license.
