Job DescriptionJob DescriptionOverviewThe Physical Therapist in Home Health role in Chicago, IL, is an integral part of the healthcare team, providing crucial rehabilitation services to patients in the comfort of their own homes. This position plays a vital role in improving patients' mobility, relieving pain, and restoring or maintaining their physical function, which ultimately contributes to their overall well-being and quality of life.
Key Responsibilities
  • Conduct initial patient evaluations to assess their condition and establish individualized treatment plans
  • Implement and oversee therapeutic exercises and activities to improve patients' strength, flexibility, balance, and mobility
  • Provide education to patients and their families on home exercise programs and adaptive equipment
  • Regularly re-assess patients' progress and modify treatment plans as needed
  • Collaborate with the healthcare team to ensure comprehensive care and effective communication
  • Document patients' progress, treatment plans, and outcomes accurately and timely
  • Adhere to ethical and legal standards in professional practice
  • Participate in continuing education to stay updated on the latest advancements in physical therapy
  • Coordinate care with other healthcare providers and community resources
  • Provide emotional support and motivation to patients during their rehabilitation journey

Required Qualifications
  • Master's or Doctorate degree in Physical Therapy
  • Licensure as a Physical Therapist in the state of Illinois
  • Minimum of 2 years of experience in physical therapy, preferably in a home health setting
  • Proficiency in conducting comprehensive patient assessments and developing treatment plans
  • Strong clinical and rehabilitative skills, including manual therapy techniques
  • Excellent communication and interpersonal abilities to effectively interact with patients and their families
  • Ability to work independently and manage time effectively for home visit schedules
  • Knowledge of Medicare/Medicaid guidelines and regulations related to home health
  • Commitment to providing compassionate and patient-centered care
  • Understanding of safety and infection control practices in home healthcare settings
  • Capability to adapt to various home environments and address unique patient needs
  • Proficient in using electronic medical records and documentation systems
  • Strong problem-solving and critical thinking skills
  • Current CPR certification
  • Valid driver's license and reliable transportation
