Job Summary:


The ACT (Assertive Community Treatment) Mental Health Practitioner or Mental Health Professional will provide intensive case management services to adults who are diagnosed with serious mental illness. ACT team members work as generalists, but also have assigned specialties. A significant percentage of ACT clients also have a diagnosis of substance use disorders, and the practitioner or professional is expected to have, or obtain on the job, specialized skills to treat the Dual Diagnosis of mental illness and substance use disorders. Services are provided on a multi-agency team supervised by a St. Louis County Public Health and Social Services supervisor, and under guidelines established by the Minnesota Dept. of Human Services, particularly those guidelines established for ACT teams. The work environment is in both an office and community environment, including in client’s homes. There is a rotating on call schedule with every sixth week, a week of on call starting Friday at 4:30 until the following Friday at 8:00 a.m. This position can be held by a Mental Health Practitioner, or a Mental Health Professional as defined by the MN Department of Health and Human Services. Duties/Responsibilities:


• Would be one of the primary case managers/specialists on the Mesabi ACT Team under the supervision of the St. Louis County Social Service Supervisor and RMHC CSP Program supervisor.

• Accepts assigned referrals.

• Complete functional assessments, treatment plans, progress notes, and other documentation as assigned and required, utilizing assigned assessment tools.

• Coordinates and provides client services in areas of mental health, housing, employment, and other basic needs.

• Work closely with all team members, providing and accepting feedback and support, and providing services to any program clients as needed to support the client and the team.

• Establish and maintain therapeutic relationships with clients, following RMHC and ACT ethical standards and boundaries.

• Motivate clients to maximize community participation and integration within the abilities of the clients.

• Teach coping, illness management, and community integration skills.

• Utilize RMHC vehicle to attend meetings, appointments, and transport clients in a safe manner and as directed.

• Maintain a Minnesota Driver’s License, Auto Insurance, and acceptable driving record.

• Work cooperatively with the team and other agencies and providers.

• Maintain appropriate licensure requirement if required.

• Participate in required training to maintain and improve skills.

• Follow policies and requirements of the Range Mental Health Center.

• Function on the job in such a manner as to promote the mission of the Range Mental Health Center.

• Rotating on call schedule. Every sixth week a week of on call starting Friday at 4:30 until the following Friday at 8:00 a.m.

• Other duties may be assigned.


Required Skills/Abilities/Behaviors:


• Requires proficient English speaking and writing skills. Must be able to communicate verbally with clients and must be able to communicate verbally with other providers to coordinate care.

• Must be able to write treatment plans, progress notes, and reports that meet billing and compliance standards.

• Must have basic typing skills and ability to utilize an Electronic Health record.

• Must have math skills adequate to assist clients with budgeting and shopping.

• Sets and meets challenging objectives before being asked to. Follows through on all commitments, meeting or exceeding expected results.

Education and Experience:


• Training and experience providing substance use disorders or dual diagnosis services to persons with mental health and substance use disorders issues.

• Must have a valid driver’s license and have good driving record to meet agency insurability requirements.

• If have a Social Work degree must be licensed by the Minnesota Board of Social Work.

• Mental Health Practitioner as defined by the Minnesota Department of Human Services. (4-year human service degree plus 2000 hours supervised mental health experience, or a master’s degree in human services, or 4,000 hours supervised mental health experience).

• Mental Health Professional as defined by the Minnesota Department of Human Services. (Statute 245I.04 Subd 2).


Physical Requirements:


• This job requires the ability to assist clients in the community and in their homes, requiring the ability to: utilize stairways, lift to 25 pounds, teach cooking and housekeeping skills by demonstration as well as verbal direction.

• Must be 18 years of age.

Requirements:
