Human Touch Home Care is looking for a Physical Therapist to join our team. We value our employees like family. We are a locally operated home care company that has been helping our clients to live their most healthful and fulfilling lives at home in Grand Junction (Mesa County). We are seeking someone to provide skilled Physical Therapy to patients in their homes in accordance with the physician's orders.

SIGN-ON BONUS $2000

Responsibilities:

· Provides services in Physical Therapy support in accordance with the physician's plan of treatment. Accepts only clients referred by the Administrator/DON

· Preform as case manager of single discipline therapy patients

· Assists the physician in assessing the client's level of functioning and development (and revision, as necessary) of the plan of care by applying appropriate tests for Physical Therapy

· Records and reports the client's reaction to treatment and any changes in the client's condition to the physician, Administrator/DON and/or case manager

· Instructs and advises the client, family, and other health team personnel in the phases of occupational therapy in which they may assist the client

· Recommends additional care and treatment when necessary

· Participates in once weekly case conference or in-service

· Conducts LPN and CNA supervisory visits as assigned

Qualifications and Skills:

· Bachelor's degree required

· Minimum of one year of experience as Physical Therapist

· Home health experience preferred.

· Must be licensed as an PT in the State of Colorado

· Must have reliable transportation and company-required auto liability insurance.

· Willing to travel within 20 miles radius.

· CPR Certified.

Job Types: Part-time, PRN

Pay: $75-90 per visit (typical visit 1 hour) - Part-time, PRN

Schedule: Day shift- Day shift / NO on-call or weekends

· Grand Junction, CO: Reliably commute or planning to relocate before starting work (Required)

License/Certification:
