Location: Office
POSITION OVERVIEW
Responsible for ensuring appropriate delivery of services and coordination of care within the department and external entities involved with participant’s care. Oversees departmental data collection and reporting activities.
ESSENTIAL JOB RESPONSIBILITIES
- Establishes partnerships with medical and mental health providers for referral source for current and participants exiting the program
- Monitors participants being connected to a medical home/primary care physician
- Receiving and answering emails and telephone calls
- Scheduling appointments as necessary
- Assists with collection of data and statistics and contributes to monthly, quarterly, and annual reports
- Maintains adequate medical supplies and inventory
- Submits daily discharges to the nurse to ensure medication orders are discontinued
- Functions as a liaison between behavioral health and medical by participating in departmental meetings
- Plan, organize, direct, and coordinate the activities of the department under the direction of the Behavioral Health Director, Medical Director and RN.
- Analyze and accurately evaluate departmental protocols, operations and services of the department and make recommendations for effective improvements
- Establish and maintain effective working relationships with participants, members of the medical team, other department staff, and the staffs of public and private agencies and groups involved in health and behavioral health programs
- Make effective recommendations regarding staffing and other personnel matters
- Coordinate with medical staff to ensure efficient participant flow
- Assist with administrative tasks such as filing, faxing, and scanning documents
- Assist with medication monitoring and documentation as needed
MINIMUM QUALIFICATIONS
- Bachelor’s degree preferred
- 2-3 years’ experience as medical office coordinator/manager or in a similar administrative support role preferred
- Knowledgeable of medical and behavioral health terminology
- Proficient in using computers, Microsoft Office Suite, and electronic medical record systems
REQUIRED SKILLS, CAPABILITIES, KNOWLEDGE
- Effective verbal and written communication and interpersonal skills
- Excellent organizational skills with the ability to multitask and prioritize tasks effectively
- Attention to detail and accuracy in maintaining records
- Ability to handle sensitive information with confidentiality
- Must be able to work well under pressure in a very fast-paced, high-volume environment
- Team-first attitude, with the ability to work with colleagues in a responsible, professional, and proactive manner.
- Strong problem solving and analytical skills
