JOB SUMMARY:

The Registered Nurse is responsible for the nursing care of the patients assigned to them and is responsible for assessing patient and family needs to promote an exceptional level of care by the Home Health Agency and provides nursing services within the scope of practice authorized by the license issued by the State of Florida for a registered nurse. The Registered Nurse provides skilled nursing services to patients in the home setting, in accordance with physician’s orders and plan of care. The Registered Nurse is responsible for maintaining compliance with agency policy and procedures, following all state, local and accrediting body regulations and overseeing services provided by Licensed Practical Nurses, Home Health Aides and Companion Homemakers assigned to the case.


MINIMUM SKILLS AND QUALIFICATIONS:

  • Graduate of an accredited school of nursing
  • Licensed Registered Nurse in good standing with the Florida Board of Nursing.
  • At least 1 year of nursing experience; home care experience preferred.
  • Evidence of satisfactory Level 2 Background screening, OIG and Sex offender registry.
  • Must have physical abilities to adequately perform described duties, must be able to lift, push or pull 50 pounds.
  • Ability to learn and apply professional training, follow physician’s orders and clinical documentation must be concise.
  • Able to see and hear adequately to respond to auditory and visual requests which relate to the coordination of Agency activities.
  • Able to effectively speak, read and write in English

SPECIAL REQUIREMENTS:

  • Conducts him/herself with the highest degree of integrity and professionalism at all time.
  • Able to speak in a clear, concise voice in order to communicate direction for Agency activities and staff.
  • Ability to organize work procedures, assume responsibility and effectively supervise others as directed.
  • Demonstrates problem solving abilities and positive interpersonal communication skills are essential.
  • Demonstrates adequate knowledge regarding exposure to bodily fluids, chemicals, some hazardous equipment as well as disease management and interventions.
  • Demonstrates good assessment skills and interventions.
  • Demonstrates ability to effectively communicate with patients, physicians, clinical manager and other staff.
  • Completes OASIS assessment within 48 hours of start of care.
  • Must have PPD completed prior to seeing patients.
  • Must complete Relias courses/trainings at the time of hire and as assigned.

RESPONSIBILITIES AND DUTIES:

  • Is responsible for the maintenance and accuracy of clinical records for his/her patients in accordance with agency policy and procedures.
  • Is responsible for maintaining the plan of care with clinical notes and filing the initial plan of care; any amendments to the plan, any additional order or change in orders, and a copy of the clinical notes in the office of the agency.
  • Assumes responsibility for each patient receiving nursing or personal care.
  • Provides nursing services, treatments and diagnostic procedures requiring specialized skills.
  • Observes symptoms, reaction to treatments, drugs, and changes in the patient’s physical or emotional condition and documents accordingly in the patients file and reports to case or clinical manager and MD as needed.
  • Initiates preventative and rehabilitative nursing procedures as appropriate for the patient’s care and safety.
  • Prepares plan of care for care provided by home health aides.
  • Counsels and educates/trains the patient and family on disease management as per the plan of care.
  • Maintains confidentiality and HIPPA.
  • Submits required documentation in a timely manner.
  • Nursing notes must be submitted within 24 hours of completion of the shift.
  • Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually as required by the agency
  • Communicates client care with other caregivers assigned to the case as needed and to Clinical Manager.
  • Reconciles medications each visit and reports abnormal findings within 24 hours.
  • Will be knowledgeable with wound care. Wound vac, IV’s and other high tech skills will be considered on the job training.
  • Disease management will be considered on the job training.

ENVIROMENTAL ELEMENTS

Employees will have to travel to a variety of patient’s homes and perform in conditions that vary greatly depending upon the client’s home environment. Some homes will be clean, neat and maintained at a comfortable temperature; other homes may be cluttered or dirty, with uncomfortable temperature.
