Position: Pharmacist Inpatient
Location: West Seneca, NY
Duration: 26 Months

Job Description:

The client is seeking a full-time NYS licensed pharmacist to provide on-site pharmaceutical services at the Western NY Psychiatric Center (RPC). This role focuses on ensuring the safe and effective use of medications for both inpatients and outpatients through careful review, monitoring, dispensing, and distribution.

Key Responsibilities:Pharmaceutical Care:
  • Review and monitor medication orders for accuracy and appropriateness.
  • Dispense and distribute medications for inpatient and outpatient needs.
  • Provide drug information and conduct medication counseling sessions.
  • Supervise pharmacy aides and other support staff.
  • Participate in pharmacy and facility-wide performance improvement activities.
Clinical and Distributive Services:
  • Conduct Drug Utilization Reviews (DUR).
  • Provide pharmacotherapy consultations.
  • Manage anticoagulation therapy.
  • Participate in health and wellness programs.
  • Deliver other clinical and distributive services as assigned.
Medication Management:
  • Optimize medication use by actively managing drug therapy.
  • Identify, prevent, and resolve medication-related problems.
  • Collect, analyze, and report medication therapy-related data to enhance patient safety and improve patient care.
Operational Responsibilities:
  • Maintain safe, effective, and efficient pharmacy operations.
  • Ensure accurate dispensing, distribution, and inventory records.
Qualifications:Licensure:
  • Must have a license and current registration to practice pharmacy in New York State by the application date.

Contractual Obligations:
  • Complete WNYPC’s Contracted Personnel Requirements prior to commencing work.
  • Agree to a Business Associate Agreement prior to the award.
