Hours: Primarily Monday through Friday 7:45am to 4:15pm

Wage: $17.00

General Description of the Position:

Provide education about the availability of the range of health services and the process for making decisions. Support people to manage and direct their own health care. Support and assist people to access all recommended health services.

Qualifications:

  • High School diploma or GED equivalent
  • Valid driver's license
  • Safe driver

Essential Functions:

  • safely operate an automobile, van, or bussette
  • move oversized and/or electric wheelchairs
  • operate hydraulic mechanical lifts on vehicles
  • use computer systems and keyboard
  • use copy and FAX machine
  • Must be able to engage in physical activity, heavy lifting, stooping, pushing/pulling items, as well as assisting persons served in behavioral/safety situations and in medical emergencies. Assist people in and out of wheelchairs and onto examination tables as needed. Assist people in wheelchair(s) on or off lifts. Assist people in and out of vehicles.

Responsibilities:

1. Schedules appointments as approved by nurse.

2. Obtains data sheets for appointments.

3. Maintains appointments on computer.

4. Prepares weekly calendar of appointments and distributes to staff.

5. Assists individuals to and from appointments assisting them as necessary and providing the health care

provider with information.

6. Obtains completed data sheets for BHW nurse review.

7. Actively participates in supporting individuals in all aspects of managing and directing their own health care.

8. Reorders medication for residential, shop, and people served by the agency.

9. Picks up medications from pharmacy on a scheduled basis and PRN.

10. Routes medication to requesting party.

11. Reorders stock supplies as needed.

12. Administers noon medication and PRNs in the absence of the nurse.

13. Obtains and documents weights as requested.

14. Repairs and obtains glasses as needed.

15. Takes orthotic equipment for repair.

16. Attends in-services as required.

17. Takes mail between buildings as requested.

18. Administers first aid in the absence of nurse.

19. Assures maintenance of nurse's vehicle.

20. Maintains strict confidentiality of health information.

21. Other duties as assigned.
