Since 1983 L.I.F.E. has been serving individuals with Intellectual Disabilities (ID) along with other disabilities in residential, meaningful day and medical day settings throughout greater Baltimore, Howard County, and since 2015 in Southern Maryland to include Charles and St. Mary Counties. We are seeking a Licensed Social Worker to contract with us for 10 hours a week. Hours are flexible but must be done onsite. This individual will report to the Director of Adult Medical Day Care Services.

General Description / Summary: The Social Worker is responsible for assisting in admission and discharge planning. Work with families and participants to access community resources. Complete paperwork consistent while complying with federal and State regulations.

Social Worker Job Duties and Responsibilities:

  • Assist with the participant’s admission and discharge consistent with state and federal regulations.
  • Completes social works summaries for all participants PCP’s. (Person Centered Plans).
  • Participates in participant’s activities and events.
  • When needed, attend the participants team meetings.
  • Accesses community resources for participants
  • When needed, processes participant’s benefits (Medical Assistance, Medicare, Food Stamps, SSI, SSA, SSDI, etc.) such that they remain current.
  • Interfaces as needed with the Department of Social Services and Social Security Administration.
  • Research possible referral sources for potential new admissions in collaboration with the Director
  • Keep the database for the participants current.
  • Conduct yearly satisfaction survey.
  • Other duties as assigned by the Director or designee

Education and Experience:

  • Must have and maintain at least a current LMSW license in the State of Maryland
  • Two years of social work experience – preferably in the DDA community
  • Knowledge of COMAR and Medicaid Regulations for Adult Medical Day Program is preferred
  • Ability to pass background check, TB Clearance & Physical
  • Local travel between offices may be required
  • Knowledge of Computers and Microsoft Office
