Position Overview


We are seeking a compassionate and highly skilled Registered Nurse (RN) with a strong clinical background to join our team as a Care Coordinator at SVT Health & Wellness. The ideal candidate will be responsible for coordinating and managing patient care plans, ensuring seamless transitions between care settings, and providing support and education to patients and families. Duties and Responsibilities

 

  • Act as first point of contact for patients within their assigned provider’s panel.
  • Coordinate preventative health care services including but not limited to well child checks, cervical cancer screening and colorectal screenings.
  • Assist in chronic disease management including but not limited to chronic kidney disease, congestive heart failure, rheumatoid arthritis, diabetes and asthma.
  • Patient care coordination including but not limited to prior/pre-authorization, appointments, medication refills, or obtaining medical records.
  • Acting liaison for other case managers and discharge coordinators to facilitate follow-through for care coordination.
  • Collaborate with team members within the organization, the Alaska Native Tribal Health system, and other referral agencies.
  • Acting liaison between specialty providers and patients.
  • Utilize patient tracking systems or clinical databases, monitor, and assure the appropriate follow-up of patients, including visiting and off-site specialists. 
  • Participate in staff/committee meetings and training as assigned by the SVTHW Director.
  • Participate in data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Patient-Centered Medical Home and Clinical Performance Measures.
  • Provide clinical support and coverage of daily functions and patient support programs/services
  • Aid in smooth patient flow while ensuring patient confidentiality, efficient time use, preservation of patient dignity and uninterrupted performance of exams and procedures. 
  • Administer and monitor medication; assist providers with medication refills/take the lead on routine medication refills using standing orders.
  • Perform procedures beyond the scope of employed LPNs or CMAs.
  • Provide appropriate patient education, addressing both preventative and acute health care needs.
  • Provide triage of patients both in-person and over the phone using protocols approved by SVTH&W.
  • Other duties as assigned by supervisor

 

Specifications (Certifications, licensure, prerequisites, special considerations)

  • Phlebotomy Training and/or experience preferred
  • Triage experience preferred
  • Written and oral communication skills
  • Ability to learn new computer programs and EHR systems
  • Ability to work as a team
  • Critical and creative thinking
  • Demonstrated ability to provide guidance and feedback to help others strengthen their knowledge in a certain area
  • Ability to allocate authority and/or task responsibility to appropriate people

Required Qualifications

Minimum: Associate of Applied Science, Nursing Degree

Preferred: Bachelor of Science, Nursing Degree

Alaska Registered Nurse License

BLS for Healthcare Providers (provided by SVT)


Preferred Qualifications

4+ Years as a Registered Nurse in a Clinical Setting

2+ Years in Clinical Case Management


Travel Requirements

Travel between Seldovia, Homer, and Anchor Point by plane, car, or boat required.
