The Family Coordinator provides one-to-one support to Transitional Age Youth (TAY) within the Youth Elemento access center and is a key member of supporting and engaging families in the city of Chelsea. This is a part-time position: 20 hours per week.

The Family Community Coordinator delivers groups, peer-to-peer support, coaching, modeling, and guidance to TAY Parent/Caregivers for the purpose of connecting to community resources that will further support and strengthen the TAY Parent/Caregivers’ parenting skills and stabilization within their community.

What you’ll do:

  • Responsible for providing and/or connecting a TAY Parent/Caregiver with resources to further support the TAY’s parenting skills. This includes but is not limited to:
    • Education on how to building stronger empathic bond with your child
    • Connect with the Youth Elemento Master level clinician to host groups that builds a stronger understanding of mental health, providing resources, and tailoring groups to the need of families.
    • Coordinate and lead on-site tours for families of young adults and highlight program areas of supports.

  • Responsible for coaching a TAY Parent/Caregiver on how to navigate and connect with community resources framed at strengthen the TAY’s stability within the community. These community resources include but are not limited to:

    • Applying for benefit and entitlement programs
    • Employment and vocational training programs
    • Education services
    • Legal services
    • Housing services

  • Responsible for assisting the Youth Elemento staff in the delivery of the daily/weekly programming.

  • Provide support to TAY Parent/Caregivers during their child’s treatment team meetings, as needed.

  • Shares lived experience with TAY Parent/Caregivers to build rapport, engage and inspire the TAY Parent/Caregiver towards improving overall goal achievement.

  • Complete appropriate documentation of service delivery as outlined by Program Manager for the Youth Elemento Center.

  • Provides marketing support in the community as identified by Program Manager for the Chelsea YA Center.

What you need:

  • Experience:
    • Must have experience as a caregiver of a youth/young adult experiencing mental health challenges.
    • Have experience navigating community resources (i.e. benefit/entitlement programs, employment/vocational programs, etc.)
  • Education:
    • High school diploma or GED and a minimum of one year of experience working with the target population.
  • Skills:
    • Bilingual (Spanish) ability strongly preferred
