Job DescriptionJob DescriptionThe Case Manager and Educator (CCME) position fulfills a diverse role encompassing intensive case management, epilepsy education, community outreach, and advocacy support tailored to children with epilepsy and their families. This position is pivotal in providing comprehensive assistance, resources, and guidance to navigate the challenges associated with epilepsy, enhance the quality of life, and ensure access to essential services and educational accommodations.

Responsibilities:
  • Provide individualized, customer-centered case management services to children with epilepsy and their families, assessing needs and coordinating appropriate social services and resources.
  • Serve as an advocate for families, addressing everyday self-management issues, ensuring adherence to treatment protocols, and securing educational accommodations in compliance with IDEA or Section 504 of the Rehabilitation Act.
  • Build strong relationships with clients, keeping them informed and engaged with EAFLA’s services and offering personalized support to meet their needs.
  • Outreach to schools and conduct educational presentations and training sessions on seizure first aid, epilepsy awareness, and self-management strategies at community events, schools, and healthcare facilities.
  • Outreach to local CBOs focused on IDD’s, such as Special Olympics, FL, STARability, Naples Therapeutic Riding Center, C’MON Golisano Children’s Museum and the Freedom Waters Foundation.   
  • Collaborate with healthcare professionals, educators, and community organizations to promote epilepsy awareness, facilitate dialogue, and ensure access to services for medically underserved populations.
  • Engage with school systems and learning resource centers to provide support and resources for children with epilepsy, advocating for appropriate accommodations and educational opportunities.
  • Input data entry into the database.
  • Assist with coordination of agency and department activities.
  • Participate in all organizational development activities.
  • Consistently treat colleagues and customers/donors/board members with dignity and respect; reflect professionalism and cooperative spirit.
  • Abide by all EAFLA and state policies, procedures, and mandates.
  • Must be willing to travel, have a valid driver’s license, and insured vehicle.
  • Must be able to carry 25 pounds.
  • Positively represent the agency.
Qualifications:
  • Bachelor's degree in human services discipline including Psychology, Social Work, Sociology, or related field preferred.
  • Ability to work independently with minimal supervision.
  • Excellent oral and written communication skills.
  • Bilingual in English and Spanish, preferred.
  • Experience in advocacy, community outreach, or related field preferred.
  • Must have reliable transportation and be willing to travel.
Salary: $45,000
