The UNM Medical Group, Inc. is hiring for a Quality Coordinator to join our Office of Population Health Management department. This opportunity is a full-time day shift opening located in Albuquerque, New Mexico. This is a hybrid position.

Minimum $42,078 - Midpoint $52,603*

*Salary is determined based on years of total relevant experience.

*Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.

Summary:

The Office of Population Health Management Quality Coordinator will assist with data validation, chart reviews for value-based contracts, and database management. They will investigate issues related to quality performance, perform medical record request/review, identify value-based care best practices, create training materials, and assist with workflow development to address quality metrics, health equity, and social determinants of health.

Minimum Job Requirements of a Quality Coordinator:

High school diploma or GED with 5 years directly related experience. Bachelor's degree with 1 year directly related experience preferred. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure (if applicable) will be required if selected for hire.

About the Office of Population Health Management:

The Office of Population Health Management supports the Health System's transition to value-based care and ensures regulatory compliance. We use data analytics to improve care delivery throughout the UNM Health System. Our team works collaboratively with multiple stakeholders to improve access, utilization, and outcomes for our patients.

The Office of Population Health Management strives to achieve the Quadruple Aim through:


1. Oversight of Value-Based Contracts and related ambulatory quality measures
2. Proactive preventative care outreach and care coordination
3. Compliance with Federal Quality Programs?
4. Implementation of population health technical solutions and workflows
5. Data driven quality improvement and reporting
6. Being thoughtful, creative, and good to each other

Duties and Responsibilities of a Quality Coordinator:

  1. Enters, collects and analyzes quality data.
  2. Coordinates the activities, data, any other items as identified to support the achievement and maintenance of health care accreditations, recognitions and protocols
  3. Monitors and evaluates appropriateness of quality control plans and practices according to protocol and accreditation requirements and state, federal health care standards.
  4. Designs, schedules and conducts quality review audits and/or clinical site visits as appropriate; performs or coordinates chart and/or other medical record reviews and records and/or abstracts specific relevant data as appropriate.
  5. Tracks sentinel events and regulatory action plans.
  6. Generates and/or coordinates the development of various reports regarding performance against quality indicators, as appropriate to the individual position.
  7. Tracks progress toward goals and ensure adherence to all required standards and requirements.
  8. Participates in the planning, development, coordination, and presentation of quality training and educational programs as appropriate.
  9. Collaborates and consults with customers regarding current quality issues and activities.
  10. Assist in reviewing and updating Policies/Procedures
  11. Assist in reviewing and updating Standard Operating Procedures
  12. Assist in conducting IP/Quality Walk Through
