Job Title: Hospice LVN
Organization: American River Hospice Services
Schedule: Shift: 2:00 PM to 10:30 PM, Monday through Friday with Shift differential offered pay

American River Hospice Services is seeking an experienced and compassionate Hospice Nurse (LVN/LPN) licensed in the state of California to join our dedicated team. The ideal candidate should have prior experience working in a hospice setting and must be committed to providing high-quality, compassionate care to our patients and their families.

Key Responsibilities:
- Provide skilled nursing care to hospice patients in accordance with their individualized care plans.
- Manage and administer medication as prescribed by the hospice physician.
- Assess and address the physical, emotional, and spiritual needs of patients.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
- Educate patients and their families on end-of-life care and symptom management.
- Provide emotional support and guidance to patients and their families during this challenging time.

Qualifications:
- Current LVN/LPN license in the state of California.
- Minimum of 1 year of experience working in a hospice setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a multidisciplinary team.
- Compassionate and empathetic nature.
