Job Overview:

We are seeking a compassionate and skilled Patient Liaison to join our hospice care team. The ideal candidate will work closely with families of individuals who are deemed hospice-appropriate but are hesitant or unable to accept the term or concept of hospice care. This role requires a unique blend of emotional intelligence, exceptional conversational skills, clear and effective communication, and persuasive abilities. The Patient Liaison will be responsible for educating families about the benefits of hospice care, clarifying the limitations of the hospice benefit, and gently guiding them towards considering hospice services for their loved ones.

Key Responsibilities:
 

  • Family Support and Education: Act as the primary point of contact for families, providing them with empathetic support and accurate information about hospice care options, benefits, and limitations.
  • Conversational Skills: Utilize exceptional conversational skills to engage in sensitive discussions with families about end-of-life care, addressing their concerns and fears in a compassionate and understanding manner.
  • Emotional Intelligence: Display high levels of empathy to navigate the complex emotions and dynamics within families facing end-of-life decisions.
  • Clear Communication: Clearly explain the hospice process, including what families can expect, how hospice can improve the quality of life for their loved one, and the scope of services provided under the hospice benefit.
  • Education: Employ education and empathy to educate families to consider hospice care, focusing on its benefits and the positive difference it can make in their loved one's final days.
  • Collaboration with Healthcare Providers: Work closely with doctors, nurses, and other healthcare professionals to ensure a seamless transition to hospice care, when appropriate.
  • Resource Navigation: Assist families in navigating the healthcare system, including understanding insurance benefits and accessing additional resources that may support their needs.
  • Feedback and Improvement: Collect feedback from families to continually improve the hospice care experience, ensuring that their needs and concerns are addressed promptly and effectively.

Required Skills and Qualifications:
 

  • Bachelor’s degree in Business Administration, Social Work, Nursing, Healthcare Administration, or a related field. 
  • Minimum of 3 years of experience in a healthcare setting, with a strong preference for experience in hospice care, palliative care, or end-of-life care settings.
  • Exceptional emotional intelligence and interpersonal skills to sensitively manage conversations about end-of-life care.
  • Strong persuasion and sales skills, with the ability to present hospice care in a positive light to those who may initially be resistant.
  • Excellent verbal and written communication skills, with the ability to convey complex information clearly and empathetically.
  • Deep understanding of the healthcare system, including hospice care guidelines, insurance benefits, and patient rights.
  • Ability to work collaboratively with a multidisciplinary team of healthcare providers.
  • High level of compassion, empathy, and patience to support families during difficult times.
     

Additional Requirements:
 

  • Must be willing to undergo background checks and meet all state and federal regulatory requirements for the position.
  • Flexibility to work outside of traditional office hours, as needed, to support families.
  • Continuous education and training in hospice care practices and guidelines.
