We are seeking a highly skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will be responsible for providing quality and comprehensive nursing care to patients in their homes. You will work closely with patients, families, and other healthcare professionals to ensure that patients receive the best possible care.


Responsibilities:

- Conduct initial assessments and develop individualized care plans for patients

- Administer medications and treatments as prescribed by physicians

- Monitor and document patient progress and update care plans as needed

- Educate patients and families on disease management, medication management, and other healthcare topics

- Collaborate with other healthcare professionals to ensure continuity of care

- Provide emotional support to patients and families


Requirements:

- Active Registered Nurse (RN) license in [Texas]

- Minimum of 1 year of experience in home health or hospice.

- Strong clinical skills and knowledge of nursing practices and procedures

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Reliable transportation and willingness to travel to patient homes in your area.

- Bilingual in [spanish] is a plus but not required.
