Intensive In-Home QP

JOIN OUR INNOVATIVE TEAM IN CHARLOTTE AND SURROUNDING AREAS AS A QP TODAY
DEFINITION AND DUTIES:
Coordinate, access, and provide Intensive In-Home services and/or any other services in accordance with wraparound philosophy and in the least restrictive setting possible to youth who qualify for Intensive In-Home services.
Act as a liaison between family, child, school, providers and the community.
Develop, facilitate and implement interventions through the child's team of providers and supports according to an individualized treatment plan.
Maintain agency records and documentation.
Respond to crisis.
Ability to work a flexible schedule in order to meet client and family needs.
Knowledge of:
- Intensive In-Home policies and procedures
- Clinical diagnosis and treatments
- Human services and MH/DD/SAS network
- Principles of choice, empowerment, and community integration
- IIH Services Definition as outlined in MH/DD/SAS
- Basic office practices and procedures including record keeping and filing systems
SPECIFIC DUTIES:
To be performed satisfactorily with or without reasonable accommodation The following duties are normal for this position. These are not to be construed as exclusive or all inclusive.
- complete the initial development and ongoing revision of the PCP
- attend the Child and Family Team for person-centered planning;
- consulting with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process;
- ensuring linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations;
- providing and coordinating behavioral health services and other interventions for the beneficiary or other family members with other licensed professionals and Child and Family Team members;
- monitoring and documenting the status of the beneficiary’s progress and the effectiveness of the
- strategies and interventions outlined in the PCP.
WORKING HOURS: Flexibility required to meet consumer needs. Some travel is required.
DEGREE REQUIRED:
A graduate of a college or university with a Bachelors Degree in Human Services with 2 years experience or BA degree in a non human service field with at least 4 years of
experience or Masters degree in a Psychology or Social Work field with at least one year of experience.
Special requirements: Must pass background investigation that satisfies Endorsement or accreditation approval. Valid North Carolina driver's license and use of personal vehicle
with applicable insurance coverage as prescribed by North Carolina law or equivalent transportation.
