Memory Care Registered Nurse

JOB SUMMARY
The Memory Partners Registered Nurse works under the supervision of the Memory Partners Program Manager and will provide expert clinical leadership, support and service development. The Memory Partners Registered Nurse will act as a community liaison to provide education, services, and advice to caregivers and those individuals with Alzheimer’s disease and other types of dementias that is consistent with evidence based best practice. The Memory Partners Registered Nurse will help ensure that services provided will exceed clinical, quality, and administrative expectations. The ideal candidate will have thorough knowledge of dementia care issues throughout the lifespan including palliative and end of life care. Must be capable of supervising other staff members in dementia care services and have the capability to interact with a diverse population to include clients, their family members, community leaders, and palliative/hospice care staff. Functions and/or duties may include, but are not limited to, initial assessment, planning, coordination, evaluation, and teaching to provide care and support to referred clients and their families. Excellent communication and organizational skills are necessary to create and present dementia care in-services, workshops, and simulation lab experiences. Performs other duties assigned.
PRIMARY JOB DUTIES
1. Identifies, evaluates, counsels, advises and coordinates with individuals, agencies or families working through dementia related challenges; works in collaboration with the client’s physician to provide needed services with the goal of life enhancement of the dementia client and family. Provides re-assessments of consults at regular scheduled intervals to maintain continuity of care.
2. Serves as a mentor to staff and community. Acts as a liaison with all contracted skilled nursing facilities assisted living facilities, Adult Day Care programs, and Senior Centers.
3. Provides community education and simulation lab experiences.
4. Serves as a resource liaison to the medical and social committees regarding dementia care services. Works closely with churches, civic groups, and senior related agencies.
5. Maintains accurate electronic documentation of care rendered according to agency policy; oversees documentation of the CNA and volunteer.
6. Organizes and performs work effectively and efficiently.
7. Maintains and adjusts schedule to enhance program performance.
8. Demonstrates positive interpersonal relations in dealing with all members of the team.
9. Maintains and promotes customer satisfaction.
10. Annually continues professional development to enhance practice and profession.
11. Effectively demonstrates the mission, vision, and values of Lower Cape Fear LifeCare on a daily basis.
JOB SPECIFICATIONS
1. Education: Graduate of an approved school of nursing, either through an AD, Diploma, or BSN program.
2. Licensure / Certifications: Current license to practice professional nursing in the State of North Carolina/South Carolina. Completes Dementia certification within six months and maintains yearly recertification.
3. Experience: Experience preferred in either Home Care, Palliative, or Hospice nursing with a strong background in dementia, Alzheimer’s, or memory care.
4. Essential Technical / Motor Skills: Must be able to meet physical demands of the job, be comfortable with the EMR, and operate cell phone. Ability to safely travel throughout demographic areas.
5. Interpersonal Skills: Works and communicates with internal and external clients to meet their needs in a positive manner. Commitment to quality service. Demonstrates responsible behavior and attention to detail and aligns behavior with the values and goals of the agency. Motivated, self-disciplined, and flexible.
6. Essential Physical Requirements: Long periods of sitting, standing, walking, or stooping. Must be able to safely lift greater than 30 pounds.
7. Essential Mental Abilities: Excellent organizational skills and prioritization of dementia care consults required. Must be able to handle pressure and deadlines, utilize good nursing judgment, work independently, and follow policy and procedure. Ability to organize and complete work requirements on a timely basis.
8. Essential Sensory Requirements: Sensitive to environment; ability to see and to speak clearly in order to communicate and perform basic job functions. Hearing and vision needed for driving and accessing client/patient residence.
9. Exposure to Hazards: Follows OSHA guidelines for tasks that involve potential exposure to blood or body fluids. Ability to withstand allergens such as dust, smoking products, and fumes. May be exposed to various animals and traffic hazards while making home visits. May encounter extreme temperatures and weather conditions.
10. Other - Hours of Work: Exempt status with flexibility to meet the needs of the department.
11. Population Served: All patient populations from infants through geriatric with life limiting illness.
12. Driving: May be required to drive. Must have reliable transportation. Will be subject to the Safe Driver Policy.
