As a Home Care Coordinator, you will carry out our Mission of Love by leading day to day operations of MVLH Home Care providing service to contracted clients. Oversees the staff schedule and payroll for the Masonic Village Home Care program. Coordinates and arranges for staff to visit clients assuring the physician's orders and client requests are met. Provides on call scheduling coverage, as assigned, to accept calls from clients or staff regarding scheduling or care related needs. Produces various reports, including unverified services, payroll hours, and monthly visits. Provides social services to Personal Care and Independent Living.

Our Home Care Coordinator is responsible for the following:

  • Oversees scheduling visits, making changes as necessary and notifying aides and clients/families of schedule changes. Oversees payroll processes for the Home Care department.
  • Follow Masonic Villages Home Care policies, as well as applicable Federal, State and Local regulations governing home care services.
  • Willing to cover 24 hour on call rotation responsibilities as needed.
  • Participates in data collection, analysis and performance improvement projects. Assists with organizational compliance with legal, regulatory, and accreditation requirements.
  • Assists clients with homemaker, companion, and non-professional care and services according to Masonic Village policies and procedures.
  • Demonstrate the ability to access and identify client care needs and coordinate plan of care based on identified needs.
  • Select, interview, train/orient and assign home care staff.
  • Demonstrate ability to market the program for future clients.
  • Assist in managing conflict and complaint management/resolution and supervises staff in performance of their duties.
  • Meet with clients and families, when called upon, to explain private duty services.
  • Oversees and communicates changes in client condition according to notification guidelines.
  • Oversees completion and review of end of month reports and other necessary reports to verify accuracy of and completeness of data entered.
  • Oversees scheduling, billing, and payroll according to department guidelines.
  • Supports collaboration with agencies and vendors for effective management of services.
  • Assist clients in completing tasks as outlined by their service plan, demonstrating the ability to assure needs are met in a safe, appropriate manner according to Masonic Village policies as well as OSHA, federal, state, and local regulations.
  • Accurately documents client information and services delivered according to the established service plan and Masonic Village policies and procedures.
  • Maintain confidentiality of privileged information, discussing, and releasing information only to those who are authorized.
  • Assist with assigned duties (including but not limited to answering phone calls, assembling admission packets, entering referrals, filing medical records, education, interviews, etc.).
  • Provides social services to Personal Care (PC) and Independent Living (IL).

What You Have:

  • High school diploma or equivalency required.
  • Two years of Home Care experience required.
  • Two years of home care scheduling experience.
  • Two years of experience in using computerized scheduling programs.
  • One year supervisory experience.
  • Valid state driver's license and current vehicle insurance.
