School Liaison

As a School Liaison, you will provide a vital link between school-based health centers (SBHC) and the outpatient/ambulatory setting at One Health, promoting awareness of the available SBHC services to students, staff, and community members and helping them access these services. You'll be part of an integrated care team that involves, among others, the patient’s primary care provider, nurses, and mental health providers in the SBHC. COMPENSATION: $18.13-18.99/hr DOE
EMPLOYMENT TYPE: Full-Time
AVAILABILITY: Mon-Fri, flex for some evenings and weekend hours for school events as needed
MINIMUM JOB QUALIFICATIONS:
Education/Certificates/Licenses:
Required
- High school diploma or GED
- Valid driver’s license
- BLS certification upon hire
Preferred
- Health-related certification
Experience:
Required
- 1 year health- or outreach-related work experience
- Computer technical proficiency in Microsoft Word and internet applications
Preferred
- Experience in diverse culture, including indigenous cultures
- Experience in care management of youth and their families
- Experience in a school setting

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

- Works with Program Director, schools, and the One Health Marketing, Engagement and Communications (MEC) team to promote awareness of SBHC services to students, staff, and community memb
- Conducts outreach and engagement with school-aged youth and their families to increase awareness and identification of behavioral health issues and promotes positive behavioral health.
- Plans, coordinates, and executes outreach events, as needed.
- Schedules patients’ appointments with appropriate providers using the Electronic Health Records system (EHR) and ensures the appointment reminder system is selected appropriately in the patients’ accounts.
- Utilizes telehealth technology to interact with patients.
- Screens patients for common health, mental health, and substance abuse disord
- With direction from licensed provider, provides patient education about common health, mental health, and substance use disord
- Facilitates patient engagement and follow-up in care.
- Responsible for school-based data collection, entry, tracking, and reporting.
- Facilitates treatment plan changes for patients who are not improving as expected in consultation with the clinical care team.
- Ensures clinical coordination and bidirectional communication between school personnel (including school nurse, counselors, administrators, and staff) and One Heal
- Advances indigenous cultural competency through increased understanding and awareness.
- Participates in One Health and school-based health center Quality Assurance/Quality Improvement projects as needed or requested.
