This position will provide leadership and guidance to CNA's providing care in the home hospice setting. This position will be responsible for overseeing day to day, strategic, and operational improvement initiatives for the CNA discipline. Must have at least 3 years of managerial experience as a CNA or LPN, and at least 5 years of healthcare experience

SPECIFIC DUTIES

  1. Creation, Implementation, and Upkeep of CNA orientation, training, and preceptor support/management.
    • Creation and upkeep of relevant training materials, guides, visual aids etc.
    • Creation and upkeep of on-the-job training tracts including specific milestones and benchmarks throughout OTJ training period.
    • Assists with identifying and recruiting CNA preceptor candidates.
    • Assess and monitor effectiveness of CNA preceptors, observe preceptors, retrain when needed, and assist with complicated training situations.
    • Assigns CNA preceptors to new hires and arranges initial scheduling.
    • Serves as a resource to CNA preceptors to assess new hire competency and assisting with remediation.
    • Check off and validate new CNA competency prior to independently working.
  2. CNA Scheduling and Assignment Management
    • Responsible for managing FT, PT, and PRN scheduling.
    • Assigns patients to CNA caseloads and assists with making changes when necessary.
    • Assess and coordinate "non billable" CNA visits.
    • Coordinate and fill scheduling gaps for absences, census spikes etc.
  3. Quality and Standards = Establish goals, coach, develop, and monitor CNA productivity:
    • Coach and reinforce daily productivity of at least 5 + visits per day.
    • Coach and reinforce the standard of 3 x visits per week per patient. At least 1 being a CARES visit.
    • Each patient encounter is at least 1 hour.
    • Coach on techniques for extending visit duration and time management for frequencies.
    • Ride along and observe for Meaningful Visit and MVI initiatives.
  4. Compliance
    • Monitors CNA documentation practices and coaches on best practices.
    • Spot audit plans of care to documentation.
    • Reports patient care issues to managers or relevant leaders.
    • Assist with variances as needed.
  5. Time and Attendance Management
    • Responsible for monitoring and approval of timecards.
    • Responsible for occasional auditing of mileage and timecards.
  6. General
    • Assist with establishing agenda and executing monthly CNA meetings.
    • Assist with coordinating shadowing opportunities.
    • Assist HR team with outreach to CNA programs and identifying potential sponsorship applicants.
    • Assist with mitigating IT/EMR issues.
    • Continue developing CARES programming.
  • Be flexible, organized and work effectively and professionally under stress.
