Job Summary:
The Hospice Certified Nursing Assistant (CNA) supports the interdisciplinary team under a registered nurse's supervision, providing personal care to meet patients' needs and ensure comfort. Duties include following written care instructions, observing and reporting patient conditions, and documenting care. The CNA's assignments aim to ensure quality, continuity, and safety in patient care.
Qualifications:
- The employee has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state and completed a competency evaluation prior to providing patient care.
- Completed a minimum of 75 hours of classroom and supervised practical training in a program approved by the Department of Public Health.
- CNA License must be active and in good standing.
- Completion of 12 hours of annual in-service education.
- At least 18 years of age.
- Ability to read and follow written instructions and document care given.
- Understands hospice philosophies, comfortable providing specialized care to the terminally ill.
- Is self-directing with the ability to work with little direct supervision. Secure with issues of death/dying.
- Provides a calm manner when in a patient’s home.
- Has empathy for the needs of the ill, injured, frail and impaired.
- Demonstrates tact, patience, and good personal hygiene.
- Valid driver’s license with automobile that is insured in accordance with organization requirements and is in good working order.
Responsibilities:
- Perform personal care tasks promptly after receiving a patient's assignment and report any issues to the Case Manager.
- Communicate positively and ethically with patients and families, maintaining confidentiality.
- Ensure patient safety and proper use of equipment (e.g., foot stools, side rails, O2).
- Provide personal care including baths, back rubs, oral hygiene, and shampoos as directed.
- Clean nails with an emery board; do not cut or clip them.
- Perform light housekeeping duties such as changing bed linen and cleaning patient areas to maintain a safe environment.
- Assist patients with dressing and undressing, meal planning, and feeding as assigned.
- Take and record vital signs (temperature, pulse, respiration, blood pressure) with demonstrated skills competency.
- Care for and observe patient's skin to prevent breakdown over bony prominences.
- Report patient's condition and changes to the Case Manager, including awareness of interpersonal issues involving caregivers.
- Perform range of motion, transfers, positioning, and other simple procedures as ordered.
- Assist with self-administered medications as per state regulations and RN case manager instructions.
- Offer and assist with bedpans, urinals, or commodes, and measure and record intake and output.
- Provide respite for patients, families, or caregivers when on-site.
- Adhere to documentation, care procedures, and professional conduct standards.
- Participate in quality improvement activities.
- Respect patient and family environments and personal needs.
- Perform other assigned duties.
