Position Purpose:

The Case Manager for the Solutions Outpatient Program in Compton, California plays a vital role in providing comprehensive care coordination to minority and low-income individuals, including pregnant and postpartum women, as well as their children. This role is responsible for upholding a high standard of care in alignment with the Substance Abuse and Prevention Control (SAPC) and Drug Medi-Cal (DMC) program guidelines, all while embracing the JWCH mission to enhance the health and well-being of underserved segments of the Los Angeles County population.

Principal Responsibilities:

  1. Execute routine care coordination following established SAPC policies and procedures, operating within the defined scope of education, training, and responsibilities.
  2. Deliver gender-responsive and trauma-informed care coordination services.
  3. Conduct initial assessments upon client admission to identify immediate care coordination needs and develop a care plan with specific timelines for each requirement.
  4. Address care coordination challenges that have contributed to drug and alcohol use.
  5. Identify immediate barriers preventing clients from staying in treatment and provide intensive care coordination for those areas.
  6. Provide care coordination services for children and clients enrolled in the program.
  7. Meet with new admissions twice a week initially, transitioning to once a week when clinically appropriate.
  8. Implement daily retention strategies for new clients and coordinate interventions to keep clients engaged in treatment.
  9. Collaborate on care coordination with community-based providers and personnel.
  10. Be proficient in using the Homeless Management Information System and input client data as needed.
  11. Incorporate mindfulness interventions during care coordination sessions.
  12. Conduct crisis management care sessions to address immediate needs.
  13. Facilitate Life Skills Group and other assigned sessions.
  14. Perform ASAM or Problem List assessments when necessary.
  15. Administer drug tests as required.
  16. Prepare reports for DCFS Social Workers and/or Probation Officers.
  17. Participate in weekly Case Review meetings for care coordination.
  18. Complete documentation within 24 hours of providing services.
  19. Submit the weekly Productivity Log to the Evaluator.
  20. Monitor client services and ensure compliance with program requirements.
  21. Collaborate with an integrated, multi-disciplinary team, including entities such as the Los Angeles County Sheriff’s Department, Los Angeles County Department of Health Services, Los Angeles County Department of Public Health, Department of Children and Family Services, and other mental and behavioral health professionals.
  22. Fulfill the required 24 hours of training as assigned by SAPC and JWCH Med Trainer.
  23. Foster effective working relationships as an integral part of the team to facilitate the organization's achievement of goals and objectives.
  24. Maintain a positive professional image and adhere to the organizational dress code.
  25. Provide coverage for other groups and activities as needed.
  26. Discuss all patient information solely with appropriate program personnel when related to the care being provided.
  27. Perform any other duties as assigned.

Requirements:

  1. Registered Substance Use Disorder (SUD) Counselor: Must be certified by one of the agencies accredited by the NCCA and recognized by DHCS, which include CADTP, CAADE and CCAPP; and who must adhere to all requirements of the California Code of Regulations, Title 9, Chapter 8 and Title 9.
  2. Minimum of two years of experience as a Case Manager working with populations facing low income, homelessness, prior incarceration, and co-occurring disorders.
  3. Preferred: Certification as a Substance Use Disorder (SUD) Counselor and/or a Bachelor's degree in Social Work, Criminal Justice, or Psychology.
  4. Knowledge and experience with Drug Medi-Cal (DMC) and the Los Angeles County Substance Abuse Prevention Control (SAPC) system of care.
  5. Proficiency in the delivery of trauma-informed treatment and evidence-based treatments for substance abuse, such as Matrix, Seeking Safety, CBT, Motivational Interviewing, and Nurturing Parenting Program.
  6. Competence in using Electronic Health Recording Systems.
  7. Proficiency in administering assessments for substance abuse, including the ASAM, ASI, and other relevant tools.
  8. Possession of a current NPI (National Provider Identifier) and "C" number.
  9. Must hold a valid California driver’s license, provide proof of insurance, and maintain an excellent driving record.
  10. Strong written, oral, organizational, and documentation skills.
  11. Proficiency in Microsoft Office applications.
  12. Ability to handle multiple tasks effectively while working within a team structure.
  13. Maintaining a current First Aid/CPR Certification is required.
