Position Summary:
Under the direction of the Director of Outreach and Engagement, the Outreach Ambassador is a sales/marketing position responsible for generating enrollments into the PACE program. Ambassador will identify qualified referrals, complete the initial intake, assist the team with converting referrals to enrollments, and educate the community through community outreach, networking, and marketing activities in the community.
In this position, the Outreach Ambassador promotes and maintains high-quality services according to BoldAge values, PACE principles, and best practices and develops and cultivates the PACE organization’s network of business, healthcare, and community relationships by:
- Developing an outreach and engagement plan for assigned territory – a deliberate and organized approach to identifying and engaging with referral partners within the community.
- Creating and implementing a call cycle with accounts based on identified strategy.
- Identifying and prioritizing accounts according to referral/enrollment potential and devising strategies to maximize access to PACE services for eligible participants.
- Actively participating in community groups, networks, and organizations representing BoldAge PACE.
- Presenting at and engaging in community events to provide PACE education and care options for older adults.
- Cultivating and maintaining positive and constructive relationships with trusted leaders in the aging services fields including healthcare providers, senior service providers, and other referral sources.
- Establishing positive relationships with potential participants, their families, and caregivers.
- Responding timely to all inquiries and commitments.
- Evaluating and learning about the community's needs, especially older adults and their families, and incorporating learned information into community outreach.
- Communicating effectively and timely with other BoldAge team members using various tools including a Customer Relationship Management tool, Outlook, Electronic Medical Record, and other systems
- Facilitating the assessment and eligibility determination processes by identifying quality referrals and collaborating with other enrollment staff and the interdisciplinary team.
REQUIREMENTS
- Baccalaureate degree in health care administration, business, communications, marketing, or related human services field is required.
- Experience in community outreach, intake, enrollment, or marketing required. Management experience in a human services organization preferred.
- Demonstrated experience in medical or healthcare-related sales is preferred.
- Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
- Bilingual preferred
** Position requires coverage for both our Oceanport and Lakehurst New Jersey locations.

Experience in community outreach, intake, enrollment, or marketing preferred. Management experience in a human services organization preferred.
Licensure/Certifications:Valid Driver’s License, proof of auto insurance and consistent and reliable usage of a motor vehicle required.
Professional license and/or certification is preferred (NHA, RN, LSW/MSW, PT, OT, RT, RD).
