Position Title: Resource Liaison, LGH
Program: RISE
Classification: Non-Exempt/Full Time
DUTIES AND RESPONSIBITIES
The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential duties and responsibilities of the job and should not be considered as a detailed description of all the work requirements of the position. - Program implementation: provide immediate, person-centered support, resources, and referrals to individuals and families for short term crises, implementing specialized services, and advocating for clients’ needs.
- Data management: maintain a comprehensive database, financial trackers, and confidential records to efficiently track community resources and client progress.
- Coaching practices and implementation: approach work with a client centered mindset, utilizing various tools and strategies and continually pivoting and applying new methods as learned continuous professional development. Support training hosted by CAP to the community.
- Collaboration for community impact: take initiative to connect with various LGH departments, CAP programs, referral partners, and community programs to ensure an impactful approach to community support.
- Innovation: actively participate in meetings, contribute to program development and evaluation, and identify new best practices for continuous improvement.

EDUCATION/EXPERIENCE REQUIREMENTS
Requirements:
- Associate’s degree in human or social services.
- Child Abuse, FBI, and PA Criminal Background clearances prior to the start of employment.
- In compliance with LGH’s Health Care Personnel Immunization policy, this role agrees to pre-employment TB testing, and showing proof of positive titer or vaccine for two MMRs and Chickenpox, and the Influenza vaccination during the flu season, as defined by LGH.
- In compliance with LGH’s COVID-19 Immunization policy, this role must be immunized against COVID-19 such that they satisfy the definition for “fully vaccinated.” *Requests can be made for exemption because of medical contraindications or religious reasons. *
- As part of the onboarding of the Resource Liaison and/or at any time CAP personnel are providing services at LGH facilities, LGH may conduct a lawful standard drug screening in accordance with LGH’s policies and procedures.
Other qualifications to be considered:
- Bilingual preferred – Spanish/English: Fluent in both Spanish and English, ensuring effective communication and assistance in the preferred language of individuals.
- A valid PA driver’s license must possess a vehicle and be able to travel to various locations throughout Lancaster County. Have valid insurance and full-time use of a vehicle.
- Ability to work flexible schedules, including occasional nights and weekends based on clients’ availability.
