Job DescriptionJob DescriptionDescriptionGood Shepherd Hospice is seeking a compassionate LPN to serve our patients and families in the Tulsa, OK  and surrounding communities.The Hospice LPN makes visits to supplement the nursing care needs of our patients/families. 
  • Full time position
  •  Thursday at 5pm-Friday at 8am, Saturday and Sunday till Monday at 8, Monday at 5pm-Tuesday at 8am, Tuesday at 5pm –Wednesday at 8am, Wednesday at 5pm-Thursday at 8 (all shift pay for 80 hours total) 
  • Previous experience in hospice, home health, or geriatrics preferred 


Key Responsibilities
  • Responding to phone calls promptly
  • Making visits as necessary
  • Carrying out physician orders under the direction of a RN
  • Informing the physician of changes in the patient’s condition and needs
  • Communicating with medical professionals, the hospice team and others 
  • Maintaining accurate, up-to-date records
  • Elevating necessary matters to the attention of the patient care manager
  • Demonstrating strong communication and customer service skills

Skills Knowledge and Expertise
  • Minimum of one year of experience in hospice, home health or geriatric care preferred (but not required for the right candidate) 
  • Must be of high integrity including maintenance of confidential information including all patient records 
  • Works as a team player, following directions and adhering to defined care plans 
  • Demonstrated commitment to the hospice philosophy of care
  • Can successfully pass background checks 
  • LPN-graduate of an accredited LPN program
  • Current state license as a Licensed Practical Nurse
  • Valid driver’s license
