General Description:
The Medical Liaison plays a vital role in coordinating and facilitating medical services for residents within our organization. Reporting directly to the Director of Community Health, the Medical Liaison ensures seamless communication and collaboration between healthcare providers, staff, and residents. In addition to providing medication management assistance, the Medical Liaison actively contributes to maintaining compliance with regulatory standards, including Rule 116, by overseeing record-keeping, appointment scheduling, and HRST management.
Qualifications – Education or Equivalent:
? High School Diploma or equivalent. Additional education in healthcare or related fields preferred.
Qualifications – Experience or Equivalent:
? Experience working with individuals with intellectual and developmental disabilities.
? Familiarity with medical record-keeping and compliance standards.
Qualifications – Abilities:
? Proficient in utilizing technology and computer programs related to medical record-keeping and documentation.
? Ability to collect, analyze, and interpret medical-related data to ensure compliance with regulatory standards.
Qualifications – Skills:
? Strong communication skills, both verbal and written.
? Excellent organizational abilities and attention to detail.
? Ability to work independently and collaboratively as part of an interdisciplinary team.
Qualifications – Licenses / Certifications:
? Valid driver’s license.
Special Equipment / Clothing:
? Closed-toe shoes with non-slip soles are recommended.
Requirements:Essential Duties:
Coordinate Medical Services:
? Facilitate the delivery of appropriate medical services and healthcare support to residents.
? Collaborate with healthcare providers to schedule and coordinate medical appointments, including physicals, dental, vision, psychiatric, and other specialized consultations.
? Ensure timely completion of all medical appointments and follow-up procedures.
Medication Management Assistance:
? Assist the Nurse and healthcare team in managing medication administration, including medication check-in, accuracy verification, and distribution to residents.
? Conduct weekly audits of medication rooms to ensure compliance with medication management protocols and identify any potential errors or discrepancies.
Record Keeping and Compliance:
? Maintain accurate and up-to-date medical records for all residents, including documentation of diagnoses, treatments, consultations, and medication administration.
? Assist in managing compliance with regulatory standards, including Rule 116, by ensuring proper documentation of appointments, HRSTs, medication errors, and other relevant information.
Appointment Scheduling and HRST Management:
? Schedule and coordinate all medical appointments for residents, ensuring adherence to recommended timelines and follow-up procedures.
? Manage HRSTs (Health Risk Screening Tool) for residents, including scheduling, documentation, and follow-up actions as necessary.
Collaboration and Communication:
? Serve as a liaison between residents, families, healthcare providers, and organizational staff to facilitate effective communication and collaboration.
? Provide regular updates and reports to the Director of Community Health on medical services, compliance efforts, and any areas requiring attention or improvement.
Non-Essential Duties:
? Assist with other duties and responsibilities as assigned by the Director of Community Health.
HSMS Specific Duties:
? Proficient in using the HSMS (Health Service Management System) for documentation and communication.
? Ensure all medical documents are uploaded into HSMS in a timely manner.
? Train and assist staff in navigating and using HSMS effectively.
? Review and approve consultation sheets before they are uploaded to HSMS.
? Regularly update resident health records in HSMS to reflect the most current and accurate information.
